-Humankind cannot gain anything without first giving something in return. To obtain, something of equal value must be lost. That is alchemy's First Law of Equivalent Exchange. In those days, we really believed that to be the world's one, and only, truth.
Baal seems to change form, so it's possible his later form (act 5) is his true form, but it is not stated, likewise for Mephisto, it is not said whether the way he appears in the game is his true form or not, but it probably is.

Well, aren't all of the prime evils fallen angels or am I mistaken, since Diablo himself was once an angel? Would that mean that they could actually look like an anti-tyreal or something?

Dream as if you'll live forever, and live as if you'll die today. "James Dean"
As Baal dies and all the spirits swirl around him, a face appears above his head and flashes around. I think its supposed to be baals true face.. ill go take a screenshot and see

I dont know if this helps or not, Its hard to get a single screenshot of the flickering face. I could use some type of video program but I dont know any
I think his body changes but his face always seems to have that influence of Tal Rasha
So i'd say the Tal Rashas face is a definite influence over Baals face and that Baals true face is the one you see at the end. The rest of his body is probably alot more accurate to his true appearence in Hell if not 100% accurate
Edit- And the voicebox, he probably sounds more like Tal Rasha than baal in cinematic before he invades that Barbarian fortress. Then when you face him at the end he sounds... like a demon
Diablo is the Spanish word for Devil. Devil is a loose term used to describe more things than the specific Satan from the Christian religion (Lucifer). It's more like this...
Diablo = Devil which CAN = Lucifer / Satan or many other things
"Lord of the High Places. So omnipresent was worship of this wild god of the hilltop that his name was carried across prehistoric Europe. The Irish feast Beltain and the Norse god Balder are but two inheritors of this bull-horned deity’s potency. Here he is depicted on the Ras Shamra (Syria) stella, wielding thunder-club & lightening bolt spear. Represented as an upright stone or lingam, Baal was consort to Astarte since at least 1700 BCE."

Technically he had done that in Diablo 1 too. Yet he still looks different.
It's also a newer game with better graphics.
I'm sure they are supposed to be the same.

The original history of Baal.
Sorry. Had to point that out. >.>
Diablo was in his true form.
I don't know about Mephisto. I hope this helped someone.
It's the decisions you make when you have no time to make them that define who you are.
