With Bliz's anti-grief stance, pretty sure Bliz will just let you dump your equipped items to your shared stash if they really want to support something like this.
I wish if you died, all your stuff just dropped onto the ground. If you have friends in the game, they can pick it up and get it back to you on a new guy, but it should be treated the same as any other monster dying, all the loot should just be dropped.
This is my wish, if you die in hardcore, your items drop to the ground, and your character turns from HC to SC. If they have shared stashs, then you wouldn't have to worry about losing anything in your stash, so it'd work well and allow you the option to keep playing.
Like I said, I don't know to what extent recovering your items will be supported. But having to rely on others to recover them is just plain problematic and will be avoided.
When your hc character dies it's usually because you were facing something quite dangerous. If you risk getting another character killed off by that same danger, you should definitely get your items back.
But I still think there's no reason to lose your items when you die - death of the character itself should be the punishment.
Considering that items make up a major part of your character, the idea would be rather impractical in terms of hardcore gameplay.
Basically you could restart another barb (or whoever died), give him the same name as the recently expired soul and presto- you are back to where you were. Just some more time invested but the char hasn't really died.
I think Air proposed a great idea- especially if you have some uber rare item on you at the time of your demise.
I myself don't play hardcore but I am assuming that most hardcore players will not want even this...
People play hardcore for one reason: play a game with real fear of death. And in diablo the closest thing to death you will ever have is loose your uber gear.
This also increase the value of items and the fun in posses then. Diablo is about build things (character, gear, wealth). Hardcore is about build those things and try your best to not loose then. Pure thrill.

"In time the hissing of her sanity
Faded out her voice and soiled her name
And like marked pages in a diary
Everything seemed clean that is unstained
The incoherent talk of ordinary days
Why would we really need to live?
Decide what is clear and what's within a haze
What you should take and what to give" - Opeth

If im in hardcore and I die, and didn't give consent to loot, tough @#$ ?
