I liked ladder and only play on Ladder now because of the restart, a fresh new clean slate is exactly what D2 needs. I won't play on ladder for D3 though not when I first buy it. I like ladder though and I'm sure it will be in
Seeing as they see ladder as their way of "fixing the economy" every once in awhile, I don't think we will see ladder.. assuming they get the economy right.

quite honestly, if thats how they feel about it, I think it will be implimented..
why? They will never really be able to fix the economy w/o it imo...
New players will join, and no one will be playing low levels, try fixing that? I don't think it's really possible, sure their will always be a few alts here and their, but I remember playing right after a ladder reset and I was like hell yeah awesome, this game is populated, so everyone helped me level, and then it got restarted and I had the chance to roll a new alt, which was also nice.
I think it really depends on the economy. Diablo 2 had troubles with duplication, so Blizzard implemented the ladder. It's nice to start over new, but if the economy's right you'll get that feeling every time you make a new character.
The one other reason they may have implemented a ladder system is so that players can race to 99. But Blizzard has all ready stated that they want the majority of the population to be able to get to the maximum level without too much difficulty. So a ladder system doesn't really seem like it's needed from what's been released.
I doubt rarely anyone would play ladder if there was no ladder only items.
I don't think anyone plays for the ladder only items, from what I remember none of them were anything special. I think people mostly played to start over fresh, with minimum duplicates and what not. Just to get a taste of the Diablo experience we're all waiting for in Diablo 3.
I like Ladder because of starting new with everyone else rather than starting new in a place where everyone is more godly then you will ever be.

Its true, i stopped playing when i got the best gear.
AND THEN CAME MR LADDER!!! i was like zomg im not spethal i goda do eet again zomgyaface. and this was justified with itemage, though everyone knew the real reason, i would not have considered it in the first place without the items (though later on having no friends in non-ladder was the reason to do it again! =3)
Twas fun, since i had friends to level with.

1 reason I like ladder: New players don't have such a hard time finding other players at all
2. It makes you want to try a new class (unlike D2 where everyone class was a Sorc to get great gear and shit)
3. Restarting gave you something to do

Ladder would be nice, actually, even if the economy works, I like the fact that everyone can restart at the same time.
