Judging by the image, I imagine the issue here is that the fire wall which supposedly killed the player is nowhere in view and I do not notice any "trap floor" beneath the toon (although my eyes are bad and the floor is relatively dark). So in essence, the OP suffered from SHC (Spontaneous Human Combustion).
EDIT:
I probably should also note that the ping appears to be fine (although usually during a lag spike, this is not reflected immediately). The player also stated that they were in the present room for an extensive period before dying; suggesting that lag is not the cause.
Dunno if that is a bug, here is a deal with that room. Whenever a skeleton walks through that fire wall he gets on fire, but the fire doesn't hurt him, instead while he is burning, he deals more damage to melee characters, something like fire aura, and its working that way since vanilla, that wasn't noticed back in the day simply because it didn't do much damage, but now in greater rift, its a different story. You can see that there is a enemy on fire.
Even if this is what is occurring, I would still say that a bug is present. If the mob is simply being buffed by an effect (rather than the effect directly interacting with the player), why should the death message reflect that the fire wall has killed the player, when it would be more correct to say that the mob has killed the player. When I kill an elite, it doesn't say "buff/debuff X has killed <insert champion pack name here>" and nor should it.
I have recently seen the "ghosted" mobs after they are killed, but they atleast seem to be pacified and don't chase you around the map inflicting damage, they just stand stationary now, well so far...
Ahh.. the skeleton on fire. A bit misleading though. Should have just displayed "Killed by Blue Skeletor" or something XD

I was fighting them in that room for like 15secs
I've seen this bug before and yes it's annoying. It pissed me off and I almost ragequit because of it.

It's been around since release. I'm very surprised no one has reported it or that it hasn't been fixed or addressed for this long.
