Game ran perfectly until patch 2.4.2 came out. Now it frequently pauses and glitches while everything being displayed, except the ground or other surface where the action is happening, disappears. I once got a message "initialize D3D error" but mostly it is "Settings: Application DiabloiIII.exe cannot access graphics hardware." This message also appears in my Windows notifications area.
My rig is CyberPowerPC gaming desktop with Windows 10 home, Intel i7-6700 @4.00 ghz (liquid cooled, not overclocked), 32 gb RAM, Nvidea GeForce GTX Titan X, and g-sync monitor running native 2560x1440@60 hz. The video driver is up to date (8-15-16 release) and optimized for Diablo.
This computer was purchased in January, 2016. I did open it up, almost no dust, removed and reseated RAM. Didn't do video card. Yes, Windows 10 automatically updated itself recently. I opened the game bar and unchecked the box per the instructions in Hayabusa's link. Will now play game and see if problems persists.....

Any ideas?
I would recommend you open your PC, clean it from dust, take out both RAM and Graphic card, clean the connectors.
Put it all back in and try to see if it works, if it doesn't also try the following:
