Before 1.04 I used to use the Bul Kathos ring with 77 Str, 148 LOH, 23% Crit DMG, a socket and 10% MF.
Post patch I went out of my way and bought a new version of it with pretty much the same Stats for 888k off the AH. It is currently equipped on my Barb: http://us.battle.net/d3/en/profile/renesco-1738/
The great improvement is that new ability it has, Life Leech mobs around you.
When first i equipped it I was in a multiplayer game and i saw 4k-8k dmg ticks when i went near mobs and was very excited. But then i went on solo runs and saw that the ticks where sometimes as low as 25 hp...
Trying to figure it out i went and stood near one of those hearts that needs to be killed after spanking naughty Cydaea,s ass to be able to better calculate the damage the ring does.
Based on whatever the full health of the heart is and the Damage ticks i was seeing i calculated that it ticks for 0.025% of enemies CURRENT hp, nd it seems to crit based on your crit hit chance and crit dmg as well.
THe health gain however is affected by the diminishing returns that are set on the Life Leech stat on the inferno difficulty.
Later on i noticed that the effect stacks when multiple mobs are around you, but given that the range for its effect is so small you really need to be surounded very closely by many mobs to notice.
So the damage ticks on all the mobs around, the life returned to you is added up in total and then the diminishing returns applied.
To cut all this short, the ring does 0.025% current mob HP in dmg to all mobs at 0 range from your character. Returns that amount of life to you added to your HP/Sec regen rate, but is affected by the diminishing returns that apply to life leech.
My evaluation is that this is deffinately a very nice ring to use on your characters and I would advise people to try it out with more attention.
PS in inferno A4 with a full party dmg ticks go up to 10k when mobs are full life...
SO all in all the harder the monsters are to kill, the better this ring will be.
I have the new version of the ring as well and have noticed that it may crit nicely from time to time. With good rolls on affixes it can be a pretty great item to have equipped as a barb.

Funny thing is I got one with nearly perfect rolls on everything and the 2 random affixes being socket and Int. FOR 888 K.

Eventually ill upgrade for one with a usefull stat where that int now sits..., prolly aim for crit chance or pick up radius (i hate having to moonwalk over gold to pick it up), but it is not really like it matters at all.
Just picked up my own [75 Strength, 151 LoH, 4% Life, 135 Armor, 24% Crit Chance& Socket] for 1.4m, will give it a test later on. Replaced my old one, which was pretty similar but obviously without the new bonus. I imagine its pretty sick for a WW Barb
With 24% Crit chance that ring has to be worth ONE BILLION DOLLAHS. Dr. Evil style!
