Basically, the last dungeon set in Act 1 is way beyond anything we say in Diablo 2, in terms of horror/gore. If you disagree, please explain HOW, don't just claim it's still full of sunshine and hookers. Say what you want about the actual color used, or the brightness, but you can't say they abandoned the horror aspect. It's all very much more powerful than D2 because the graphics are so much better.
It's called something like the Dungeon of Agony or something suitably cheesy, right in vain with D2. Why is it so crazy?
1) One of the destructible items is called "Basket of Heads." It's exactly what it sounds like. If you destroy it, human heads go flying.
2) Among the torture instruments is a Rack with some form of spiked racks at the bottom. If you destroy it, a corpse gets squashed by the rollers - along with plenty of squirting blood.
3) There is a version of the Sarcophagous from D2 that spat out mummies. Instead of mummies, it's undead. It's called the Iron Maiden. It's not so bad, except the first one you see is open, with a living captured villager in it. Then it closes. With the accompaning visuals.
4) Towards the end, there is another of the ghost cutscenes, like when Lachdanan kills Leoric under the Cathedral. Only this time, it's Leoric, Lazarus, and Leoric's wife, with Leoric and Lazarus beheading her with a guillotine. Complete with squirting blood. It's very chilling.
