I talked about this with a few friends and we were also unsure, but had a few scenarios:
1 - this is a reference to a large patch update announced at blizzcon.
2 - A new system that will be much like trials, except it will operate in surviving as long as you can for a certain reward (like a crucible basically).
3 - A new type of dungeon system? other than a crucible
4 - Just a T-Shirt that looks cool
We had a few other drastic ideas, but I shared the ones I think are most likely.
I also wanted to note, but the image icon looks like tathamet, who is also present on the very first cinematic where the angels were fighting the demons.
I personally felt like it would make sense to include him in the game somehow, and maybe now that the black soulstone has been shattered, he was able to reform (he was a combination of all the demons as one entity).
Besides that shirt, the Key Art for Blizzcon 2016 features a version of Diablo that differs fromt the one seen in Diablo III (it's the Male version for a start).

Hype or NOT?
If it is not tathamet (which is what it looks like to me) it certainly could be the return of Diablo that Adria talks about when we confront her in Act5.
Plus, we still have to see the fall of the Nephilim, which was all but promised to us, and that does not make sense as part of the D4 story in my eyes. I do not think 2.5 is going to just be a big content patch, I think it might be an expansion worthy patch, This would be optimal for the company to bring back as many fans as possible, because the prestige that Diablo once carried with it has been waning fast.
I do not think simply releasing D4 would be enough to bring back the masses, but maybe I am wrong on that.
All we can say with certainty is that this shirt is clearly Diablo 3 related. Maybe this is the name of the next expansion? I would be shocked if they went the expansion route, but if they are truly bringing back the good ole group of Blizzard North, they can really turn D3 into one of the most epic experiences EVER, all while biding time to make D4 that much better than D3.

You have a point. D3 is not as old as D2 to give up on it. It has a lot of lifetime ahead. So maybe we will see a 3.0 indeed.

Anyway, the t-shirt's name clearly references something. It's likely not D1, D2 and D3 together; the t-shirt is clearly a D3 specific t-shirt, and beyond that... all three games are mainly set in Sanctuary, with parts in Heaven and Hell. Only one of those could be considered a "realm of evil".
There's potential that "Realms of Evil" part refers to the three realms of Hell ruled by the three Prime Evils - Diablo, Ball and Mephisto. This would run in line with the triangle depicting three distinct "runes" at each point ( perhaps someone can look into those runes, to see if they conform to the three Prime Evils in some way, shape or form ). They aren't D2 runes ( *checked* ), so it's not a runeword.
Also, and interesting point to note is that the Diablo depicted on that t-shirt is *NOT* the same Diablo we defeated pre-RoS. It's the male version, implying that he's LOST traits he attained after he "destroyed"/"morphed from"/"whatever from" Leah. So... yeah... no idea what the means, but in a voice in my head ( possibly Diablo's ) is saying it lends some credence to the idea that he may well release the other Prime's back into Hell ( lending a good reason why we'd have to trawl through their... "Realms of Evil" ).
So let's suppose it IS an expansion for one second:
We know that after the events of D3: RoS, Diablo is released with the destruction of the Black Soulstone. He still contains the essences of the other two Prime Evils - Baal and Mephisto. We have no idea what happens after that, but ( and this is entirely supposition )... it would be fairly easy to say that... facing an empowered Nephalem that has defeated many of his primary allies/servants, he fled back to the safety of Hell from Sanctuary, where he embarks upon a new and final plan to destroy the Nephalem and finally conquer Sanctuary. My guess would be that he CHOOSES, for whatever reason ( three heads are better than one, I guess )and via whatever means, to release Baal and Mephisto, forcing the Nephalem to once again battle through each realm of Hell, in a "final" (citation needed...) war against the three Prime Evils.
I'm not sure it IS an expansion, but that's where I'm going with it if it is. And it might be. Blizzard royally screw with us every time they announce something like this, and given that it is the year of Diablo... they're almost certainly screwing with us atm.

That is where I was drawing a lot of my conclusions from as well. It seems like the logical story, the question at hand is whether it is expansion worthy, patch worthy, or new game worthy. But I feel it is something we will find out about, otherwise what is the point of even showing that battle in the beginning of D3?

Blizzcon schedule suggests new content is on the way, maybe the book offers a hint of what it might be.
What does it say in the book? Anything that could a potential story for another expansion?
According to this "Burning Hells - Realms of Evil" book chapter, the so-called Netzem Petroglyphs were found on what is believed to be a natural pillar far out in the deserts of Aranoch. They were discovered by Deckard Cain and Leah. Upon staring at the rock, Leah began having visions of a most violent nature, and Cain had to confine her to the tent for the rest of their time there. Possible scenario (speculation):
- The Neztem Petroglpyhs mentioned in this Realms of Evil book chapter are a direct connection to the Burning Hells
- Diablo took posession of Leah when she and Deckard Cain examined the stone with the glyphs in Aranoch
- The pillar or the glyphs are the key to Leah's redemption, the theme of the next Diablo 3 expansion (or huge content patch)
- Leah's soul (and probably that of Cain and others) will be redeemed
- Metzen is redeemed, as is "Neztem" a reversal of Chris Vincent Metzen's surname

I'm leaning towards patch 2.5, BUT: the Blizzcon panel sheet speaks of "first look at their plans", so the chances for D3X2 are there.
They could also go the way of DLC if the content is sufficient enough. Would have to have a story based element aka a new act.
I'd love it they released a mini paid expansion that introduced a new act, 2 new classes (one that specialises in all main stats and the other in vit as their main stat), doubled the stash size to a max 20 tabs (for those of us who unlock all tabs via seasons), as well as throw in 10 additional character slots. Don't particularly want a level cap raise. They can then just keep churning out content patches every year then.
I have the CE with with 10 more character slots, that's 25 slots... if there were 8 classes, that's 3 characters per class in my permanent roster I could rotate between and 1 more I could use as a disposable seasonal character... just to get the cosmetics.
And then they can have David Brevik and Bill Roper work on Diablo 4 for a few years, before we migrate away from D3.
I feel we are getting an expansion or a brand new game in the Diablo series. Leaning toward brand new game.
Why not getting both? The other game could just be a Diablo spin-off built on the OW engine (like Hellgate London with better graphics). If something like this comes up we could see a potential release in 2018/19.

If I were making the decision, I feel the resources would be best spent on a large content patch that overhauls some of the major D3 issues, and primary resources on our new project.
But Blizzard often times is very generous, so maybe we get all THREE (Patch, Expansion, New Game)!!!!
