Here's another thought: Inferno mode isn't ready yet, and it is what will be part of the first major patch of the game ~3-5 months out from release. That would explain their denial/lack of explanation about the interview. Then again, they deny everything that everyone says, even if it's 101% accurate. (The extra 1% are those pesky "undocumented changes" we all hate that come with every patch to whatever game)

i thought about the inferno level (not 100% confirmed)
i think it will be amazingly cool if the inferno level is granting you a considerable amount of MF and rare availability but with the cost of "REAL" death.
in other words:
i wish blizzard will make inferno deaths function as hardcore death.
this way you'll get a very exciting tradeoff between MF end game and caution -the fear from dying.

Blizzard is not stupid, so they wont do something that people will not appreciate from what they have conceived as a very awesome casual game.
in D2 some high end players still played nightmare difficulty runs.
that means that not anyone aims to play hell.
here its the same: inferno is not a MUST.
its optional, its always there, not an obligation. it sits there and you know its there.
that adds a new level of excitement.

Ever thought why Blizzard still hasn't confirmed the fact... it's because, it is a secret.
Inferno may be a secret level that will only appear as A NEW CHOICE FOR those PEOPLE WHO FINISHES THE HELL DIFFICULTY level...
meaning, the Inferno difficulty option will only appear AFTER you have beaten the Hell level.

I doubt that, since people will hit the level cap fast, the reason they haven't commented on it is because they don't wish to discuss the end game yet, I'm sure the end game will be one of the main topics at blizzcon because it will determine the replay value of the game.
As long as they stick to what they've said earlier about being able to complete everything solo, I'm all for a fourth difficulty. When they start making things impossible for one person to complete, it isn't an incentive for me to play coop with others. It's more of an incentive to stop playing. And it's the only thing I can think of that could ruin my enjoyment of the game.

I was hoping that there would be some joke areas in inferno difficulty, if not in all the difficulties. But with inferno difficulty they could make it ridiculously hard, which sounds like it'd be a lot of fun.
I was kind of hoping Inferno difficulty would be perhaps one large and very difficult multi floor dungeon, perhaps as a tribute to D1.

Yea, I don't thik the cow level in its purest form will be returning. Basically I mean the cow level was used to grind those levels and because we are to be finishing the game at about max level I don't see that type of secret level returning. However, maybe some other secret level. I think it would be cool if a secret level was heaven or hell.
