I just noticed these two seemingly unused pieces of concept art from Diablo 2, and thought since the inclusion of the last 2 Lesser Evils in Diablo 3 is potentially on the table that I'd bring them up. I have no current proof they could even be what I'm suggesting they could be, but I wanted to toss them up for opinions.
No, it probably isn't Belial or Azmodan, especially since the guy to the right resembles mephisto (lanky, boney, tall) and the one on the left looks very similar to the discriptions of Baal from the original Diablo 1 manual (there was no actual detailed discription, though they did give you some sort of idea of what he was like)
The strange thing is, the one three-headed Demon in the trailer for some reason says to me "that's Belial." I have no idea why but I feel like I've seen that image before from somewhere...not the exact one from the trailer of course, but that depiction of Belial being a firey balroog-like demon with 3 heads.
I'll have to look into it...it's been bothering me for a while now

We've only seen Soulstone corrupted versions of the Prime Evils, not their true form.
Yep, the last time the Prime Evils were in their own bodies was about a thousand years ago. No mortal since has seen Diablo, Baal or Mephisto in anything but corrupted human shells that are manipulated to look sort of, kind of, almost like their real forms.

So we know we have seen Diablo's true form. As for the other two it is unclear if Mephisto is in his true form (he could be, there is little evidence to suggest otherwise), and as for Baal. He is missing his soulstone and thus part of his power. His form in LOD could be his true form, but it is just as plausable it is some sort of mutation or corrupted for of Tal Rasha.
The whole thing with Tal Rasha being both the host and the soulstone creates a lot of headaches for people trying to better understand the lore.

Considering I couldn't even connect them until he said that, I just have to say that I'm incredibly impressed at how far designs came from concept sketches.

Paradoxically, some apocrypha credit Belial as being the father of Lucifer and the angel that convinced him to wage a rebellion in Heaven against God, and that Belial was the first of the fallen angels to be expelled.

Earth - As oppose to fire in the artwork.
In the Diablo video game series, Belial is one of the seven Great Evils. He is never seen in the games, but is described in the first game's manual as the Lord of Lies.
Lies - Consistent with "angel of confusion?" Maybe..
In the video game Devil May Cry 4, one of the early bosses is "Berial" ('R's are interchangeable for 'L's when translating from Japanese). He appears as a flaming centaur and introduces himself as the "conqueror of the Fire Hell."
Centaur - The artwork is definitely not a centaur.
Also, don't bother google imaging "belial." There are hardly any results that are on topic and of those they aren't very similar.

I think diablo and mephisto were in their true form in Diablo 2 or at least diablo as they tell him to were his true form in the Cinematic right before you enter the pandamonium fortress... For Belial and Azmodan, i think they killed each other after the sin war as it says that they were rageing war upon each other in the book "the sin war" in diablo 1. the tome "the sin war" says that they overthrew Mephisto Diablo and Baal and after that they were fighting with each other about who should have the power... or at least that is as i remember it..
