Died a few times. First was at Butcher in act 1, didn't have that much HP and he basically one shot me. Rest were random deaths due to explosions and one shot mechanics.

Playing lots of the beta made me carefree. I have died a few times to particular champ packs. Especially on the final act. My second char is having a much better time though.

First time was during act II. I entered a camp with walls on all sides and it contained 3 packs of mobs, including one champion pack. This was a bit too much to handle.
Second time was yesterday evening. I tried the first boss of act IV, but failed big time. I guess I'll die some more in act IV.
I died so many times in Act III cause of those goddamn Heralds of Pestilence. They are probably the most overpowered monster in the game if you're a melee character, especially if a champion mob imbued with waller / jailer is around and manages to trap you.
I probably died at least 10 times cause of those fuckers.

I died once in the sewers in act 2. I was trying to fight this one yellow guy, and another one jumped out from around a corner with a nice little vortex modifier. At that point there wasn't much I could do to escape.
Mmm, yeah hate to admit it, but I've also died a couple of times in Normal. My first death was in Act 2 in one of those sand caverns. All I remember was that there were a champion (1 yellow + minions?) mob and right behind them a rare mob (3 blues?). I played the monk and didn't noticed the rare mobs, so I used "dashing strike" and ended up between the 2 mobs with no escape. Oh and at that time my roommate came in and started shaking my shoulders for no reason <_<. That didn't help at all!
Second time was to Izual. For some reason I didn't notice those frost orbs he was casting all over the place and I just kept on fighting and eventually froze and died...
I'm in Act 2 Nighmare and I'm proud to say that I haven't died once!! In fact I find it quite easy at this stage. Beat The Butcher without using any potions or skills...but I do have that one passive that give me life if I spend spirit.
PS Those flowers the trees make and those wasps in Act 2 that sends out all that tiny shitty little wasps at you is the most annoying and overpowered. Almost died there a couple of times though.
Hmm... yes, I've died a number of times. I'm still acclimating to D3 as opposed to the mechanics with D1 and D2 (rapid fire potion dispenser? yes please). And to be clear, I didn't play the beta, so all of this is first time stuff for me.
I started with a WD. When I got to the event with the four columns of ever-spawning skeletons, I died. Repeatedly. So much so that I quit (not ragequit tyvm) for the evening (I don't get a lot of playtime in the evenings anyway).
The next time I tried, I switched from throwing JARS OF SPIDERS to the rapid fire poison dart approach, which made a huge difference. In fact, it was amazingly simple at that point. It made me question why JARS OF SPIDERS were even on offer to be honest.
My next roadblock was the Skeleton King. This time though had everything to do with hardware issues. My laptop is a little old and doesn't have a top of the line viddy card. I had it set to max resolution but min textures, shadows etc. I had noticed walking through that things got a little pokey in certain areas and things seemed to bog down. Then when I engaged the SK, he ate my lunch, then laughed at me and stole my allowance money too. It was tragic. So I've backed down on the resolution to the point where the game runs smoothly and I was able to defeat him. I did make an attempt with the spiders again but to no avail.
I also learned that, unlike with my experiences in WoW, stuns and snares and the like DO affect bosses! Yes, at first I wasn't bothering to try to CC him because I figured, hey, boss, it won't work. You can imagine how much easier it got at that point too.
So I got past the SK on my WD and started leveling the other four classes past the SK for comparison.
Barbarian was just easy. Flat out. I didn't die, got to the SK and ate HIS lunch. Which was kinda gross given what he eats these days. Ick.
Monk... oh, monk... the first time out, I died on the "Kill the Puking Dead Chicks What Spawn the Zombies" thing. You know, where you kill 3 of them and then go find the fourth Queen version? He died at the city gate. I switched heroes at that point. Figured I needed to let that simmer a bit before I returned.
Next up was the DH. Interesting experience. It was at this point that I discovered that if you hold your mouse button down on a target, you will continuously execute the indicated attack. Yes, IRt3hn00b. Anyhow, I died a few times before I realized that the playstyle here is required to be more mobile. Where the WD does require some mobility, I ran with demon dogs a helper with Cormac and sometimes boatloads of spiders, meaning that in many cases I had a wall of bodies up front keeping me from harm. With the DH, especially early on before you got Cormac, I started realizing I had to use Caltrops and give space in order to take down enemies, especially if they are in a large pack or include champions. I also began to become more reliant on the "spending" abilities. Oh dear god I loves me some rapid fire. Anyhow, SK wasn't too bad this time around and I got through without what I felt were an excessive number of deaths.
Died again on my monk.
I then started my Sorceress. Having learned valuable ranged lessons on my DH, I was versed in trading space for damage. I saw I had things like Ray of Frost, with a slowing affect. "Aha!" says me to myself "I shall slow my enemies with quick bursts of frosty pain and then strike them down with magical missiles" Except they were like "Hmm... let me slow down for about 0.0125 seconds.. aaaaaand back to chasing down the sorceress." So that was a bit annoying. I used potions more frequently. I agree with those who say that, early on at least, the sorceress feels a tad weak. I was really miffed at how weak MM felt until I got the first rune for it. I also wasn't clear on how my weapon could affect my spell damage. As I started learning that lesson and when I got my rune, my sorceress started feeling a bit more peppy. I got a few decent bits of armor and didn't feel quite as squishy. Beat the SK with no real issues and was good to go.
Back to the monk. Miracle of miracles, it felt easy this time. No, really, I was confounded as to how much easier it felt. No difference in gear. Just suddenly felt easier. As I leveled, I opted for survival options. Using the Breath of Heaven heal ability and stuns, SK was pretty easy actually. Perhaps the easiest of all the bunch. I felt so confident that I opted to take the Monk forward past SK. Things were pretty cakelike. And I like cake. So I kept going until I hit the Highlands above Wortham. I ran into some sort of Khazra encampment. There were three named critters. Also, before the evening was done, there were a handful of my corpses. Or would have been if the game allowed such things. In fact, the first time I hit that encampment, I went down so fast it was ridiculous. In fact, honestly, I didn't really beat that group so much as wear them down in a war of attrition. It was ugly and I was bad and I felt bad. After that.. well.. something else happened and I don't remember what. I did beat the queen spider chick but ... *sigh*... when I quit last night it wasn't because I had had a fulfilling night of monk-time. It was because I had bashed my face into a wall of some kind and thought "You know, I think I would rather just go to sleep than actually try to defeat this obstacle." Sadly, I don't remember what it was. But the tone was definite not of the conquering hero.
So I have died.. repeatedly. I'm still getting used to how to play each class, how to balance offense and defense. I wish there were an easy way to store configurations of skills/runes and quickly pick between them, like a weapon swap. I wish the spiders and toads felt potent enough to use them consistently. I think that, if nothing else, I feel like I'm not as good a player as I thought I was given how much I've died. I also feel like it means I'm not doing enough research and/or that there's not enough information in game to help you not screw up on things like gear and skill selection. Still, I'm having fun and enjoying the story.
I think i might have died once in act1...dont remember, possibly not though.
First time i remember dying was to the wasps in act 2. several times. towards the end of act 3 and early act 4 i was dying left and right as a barbarian. Then i realized i was skimping on vitality and farmed up/crafted some gear that gave me about 150 more vitality doubling my hp, and sacraficing some damage in the process. I was already doing insane damage(when i wasn't dead) so the hit to damage wasn't noticable. Now that i'm aware of how important keeping a high health pool is, i'm steamrolling through nightmare, cleared act1 without dying or even being concerned once. I've also changed up my build a bit as well to include a lot of self healing / mitigation.

Small update.
I finished normal last night without any additional deaths. I swapped some skills, went back to a previous quest to try them out and when I felt comfortable with those new skills I started act IV again and finished this (mini-)boss within 10 seconds.
Azmodan's fireball 1hit ko my witch doctor because I didn't think it would kill in 1 hit.
Yeah. That was quite the shock for me too. On the barbarian, that attack does nothing. I was like, whoa! Azmodan fight a ton harder on the WD than the Barb. Longer too. That was my only death on normal, I think. Belial took me more tries than I'd like on nightmare, mainly cuz I'm stupid. But this is expected.
I'm playing my WD now. Act 3 nightmare. It's a -ton- easier - took no damage during the Ghom fight woot. Haunt rocks - fires over "waller" walls, lets me target a dude 8 miles away thru a crowd of other things. It's like the best spell ever. Only mob I had any sort of problem with was teleporting/fast shield demons. Suckers. They 1 shot my dogs - generally I keep them alive by gathering up globes I don't need to use. They last pretty good, despite peoples's complaints. And the big dude he never dies. He can take a pounding.
And I'm able to stack MF/GF on my WD too. The barb was at 0 at this point. WD is at like 73% or something. Hoping to get upgrades for hell, since this is about where that stuff should start dropping.
In normal I died plenty of times to rare mobs with a big pack. The pack of crappy mobs would surround me and the rare mob would smack the shit out of me.
Currently in Act 1 nightmare (sword of the stranger quest) and haven't died once.
I'm not sure how this happened, but 3 (maybe 2) Mallet Lords appeared out of nowhere right on top of me. I'm not sure if they can be summoned or can appear out of a ground portal animation type thingy (if that makes sense), but they did appear out of nowhere. I don't think it was lag but you never know.
Anyway, since they popped right onto my head, it only looked like 1 Mallet Lord. I figure I can tank him but then I see my health drop to half and then to 0 really fast. Besides that, I never came close to dying elsewhere in Normal.
I've died a handful of times just because I wasn't paying attention. I see a group and run right into the middle and start wailing away. That's a good way to get killed, but I still do it because it's fun.
I'm sure I'll have to change my approach when I move on to the more difficult modes. It's going to take me a while, though, since I play maybe 2 hours at a time. I'm progressing slowly -- at L24-25 currently.

Well it certainly wasn't the monsters
My 24 Wizard died to Belial in normal too. Again I got myself cornered and got stomped on.
They are the only times I've died in normal.
