Offhand can be any Phylactery, Just make sure it has Maximum Essence as close to 20 as possible. Then follow the build guide for remaining stats.

cubed Jewelry is between Krysbin's and Circle of Nailuj's Evol. Circle of Nailuj's needs to have 4 seconds on the secondary. So if you get one with less than 4, put it on the cube. If you get a good one with 4 seconds and good rolls, but a Krysbins with bad rolls, put the Krysbins in the cube.

Diamond in Helm, Topaz in Rest of armor.

Also with this build, you don't run a generator, so you are relying on your Barb / Znecro to produce and pick up health globes to benefit your bracers.

Cooldown on as many places as you can get it. You can see in the build guide above where you should try to get Cooldown. The more Cooldown you have, the more you can use Simulacrum
