NOTE:
If playing Hardcore, use Unity for your 2nd ring slot and equip a Unity on your follower. Otherwise use Stone of Jordan, or any other rare/legendary ring with the specified stat order for extra DPS.
You can wear either Hammer Jammer or Depth Diggers in your armor slot, just pick whatever rolled better in stats.
Any legendary shoulder armor will do as long as it has the specified stats.

Build Guide
Another Thrill/Super Human build I came up with to help those who are struggling to get this conquest as a Crusader. The build was successfully tested at Paragon 237 on Hardcore with 60% of the timer left in a really REALLY bad rift on the first try. None of the items on the list have to be ancient, nor do you need any of the recommended items, Hellfire Amulet, or legendary gems. The sheer amount of damage boost to Blessed Hammer from the required items pretty much murder everything in seconds at Greater Rift level 45, so you should have zero issues pulling this off in Softcore or Hardcore.
LoN Bomber works too, but I found my way much easier to gear for in a shorter time frame.
Unless I'm missing it, there's nothing in this build to actually activate the Hammer Jammers. Or does the slow from Bane of the Trapped count?
Flurry Rune on Falling Sword as well as Bane of The Trapped will proc the Hammer Jammers.
