This build does not use Mystic Ally but instead relies on high CDR to keep Epiphany up with Exalted Soul and Sweeping Wind/Inner Storm to maintain Spirit. Without Mystic Ally you have other options for boots, I like Ice Climbers but entirely personal preference.
Some builds stay away from Vengeful Wind but IMO having 10 stacks for Sweeping Wind vs. 3 stacks drastically reduces the risk of Sweeping Wind dropping (which is where you get almost all of your damage and at least half of your spirit).
If you don't need/want Sweeping Wind on autopilot then Crystal Fist is a good option for ~50% Dashing Strike damage reduction.
For max CDR equip Zodiac Ring. (In fact you pretty much have to do this to gain perma-Epiphany while in map)
Cube Gloves of Worship for Regular Rifts, Eye of Peshkov for Greater Rifts, Leoric's Crown for either if you need CDR boost. CinderCoat RCR is not necessary for this build, freeing up other options.
Eye of Peshkov really makes a difference for this build by reducing Cooldown for Breath of Heaven by 50%. If can get around 63% CDR with Zodiac Ring you'll have Perma-Epiphany while engaging mobs + Damage Boost and Heal from Breath of Heaven every 2-3 seconds. Once you reach RG you'll have sufficient Spirit to get the job done but you won't be able to spam your attacks like you can when there is ton's of stuff to hit and proc Zodiac.
Don't cube Tso Krin's Gaze because you'll lose ~130% damage increase.
For legendary gems you'll want Zei's Stone of Vengeance for long range damage boost and Bane of the Trapped which, with Pinto's Pride bracers, massively slow down incoming mobs. For the third legendary gem, Bane of the Powerful or Gogoks work well for regular rifts, Bane of the Stricken seems to work best for defeating RG in greater rifts.
I've not had a chance to fully optimize my gear for this build yet but can easily complete GR68 vs. GR70 with my other super-optimized/Caldesanned Istvan's blades build.
The posted video is with MageFist cubed, I'll update as soon as I can to illustrate the impact of cubing Eye of Peshkov.
This is the most fun I've had playing D3 in months.

50% damage reduction would be sweet but unfortunately MageFist, Lefebre's, Peshkov and Tso Krin's all occupy the same slot in the Cube so I couldn't find a way to use the combination.
You could however equip Crystal Fist in place of Vengeful Wind for 50% damage reduction but for that to work you'd probably need more CHC to maintain your Sweeping Wind stacks. That would probably require swapping out CDR for CHC on some of your gear which would then cause Perma-Epiphany to be less Perma. (right now I've managed 44.5% CHC and 63.53% CDR) and this build really needs Perma-Epiphany.
Regarding Epiphany; it won't become Perma until you start hitting mobs with a 'resource-spending attack' (like WOL) which procs Zodiac Ring. You'll also want to 'numlock' Epiphany activation so that it self-activates.
In the final analysis this build likely falls short of the Quinsane build when pushing higher Grifts but I really enjoy this build because I don't have to time COE just right or constantly worry about Sweeping Wind.
This build is also a blast with Gloves of Worship when running Regular Rifts, especially with a group.
One other note, the high CDR of this build + Peshkov's allows you to activate Breath of Heaven/Blazing Wrath every 2 or 3 seconds and while the added damage is only 10%, the continual heal boost is incredibly useful.
More than anything else, I've just had fun with this build whereas my Istvan's build felt like a job. Actually having fun playing a game has been a welcome switch from constantly missing the GR timer by 10 seconds and cursing at what's her name because she shows up even though there is no point since you can't upgrade legendaries if you miss the timer. "Good to see you..." No it isn't, (60%, failed; 60%, failed; 60% failed. Grrrrr). :>)
