One thing I found extremely useful as a hardcore monk was the use of Mantra of Evasion: Backlash coupled with a life steal weapon. You can literally stand there in a huge group of monsters without doing anything and the life steal from the dodge explosions will keep you alive as long as the mantra lasts. Makes you kind of immune to death-due-disconnect (unless it happens in a bad spot like elite packs or boss fight like it did for me). Obviously you need decent EHP and dps also.
I would also recommend using a shield. The huge chunk of EHP on shields makes gearing your monk easier because you can ignore EHP on some other slots in favor of pure dps. In essence you dedicate some item slots for pure EHP (like shield, chest armor, shoulders etc) and some to pure dps (like rings, amulet, gloves). It's cheaper to gear like that instead of trying to find both dps plus EHP on the same item.
If you go with a shield, ideally you want a fast weapon with high dps and a ruby in the socket and a good amount of attack speed on gear. If you go with dual wield, just go like you would in softcore (crit dmg on weapons with emeralds in sockets, weapon speed not as important).
Whether you go with a shield or dual wield, decide on it early because it's easier to gear according a specific path with the end goal in mind.
