If you're unable to do that for some reason, perhaps this will help.
I used a similar build as you, but I really like hex with the healing rune. Hex healing and pots was enough cover up from any dmg from his ranged attacks, so you should be fine. If it's the teleport thats killing you (1 shot me), you just have to time spirit walk really well so that you don't get hit by it. Otherwise, just keep your distance from him and you should be fine.
This is by far the build that I have the easiest surviving with, mainly because I only have 1 dmg ability with the rest as "get the hell out of here" abilities. Timing the wall, fear and spirit walks with the pets chickens makes it relatively easy to keep mobs, and even big packs, at a safe distance. I stopped using soul harvest after hitting act 2 Inferno because the risk of dying while trying to get some stacks on was very high because of it's small range. And I really don't like blowing spirit walk, by far your best escape ability, just to get some stacks.
This build works very well for me against everything except the nagas in act 2. Haven't found a viable build to counter their invisibility being able to move through everything I do to stop them.
Darts alone works surpisingly well on big packs, especially with a sword/dagger/knife and some haste, and excellent as single target. But it's very boring -.-
