Thanks everyone that answered- the unity thing didn't occur to me. Without the invincible item, your follower is going to stand in fire, etc... and get you killed.
So always use the invincible relic item on a follower with unity.
I would agree that most of the Season 6 conquests are on the easy side. I hope we will see some more interesting conquests by the time S6 goes live. I don't mind a few easy ones to let everyone get the stash tab, etc... but I personally want some meatier conquests. It would also be nice to see the achievements as you get them (partial fix in 2.4.1 but mostly still broken).
Yeah it looks like it happens in the video first around 7 seconds and if you count the smite proc every 5 seconds it coincides with your toon's death. Definitely want to post that in the bug forum. You won't get your character back but will help this not to happen again... to you or someone else.
Make sure you post the console (PS4), the skills you are using when it happens (e.g. Steed Charge-draw and quarter, etc.), and anything else you can think of that might be helpful.

I Can't Stop- Level 3 legendary gems to level 65 in Hardcore mode.
Boss Mode- Kill the following bosses on max level on Torment X within 20 minutes of the start of the game (lists all 16 bosses). Worlds Apart- Kill the following bosses on max level on Torment X within 20 minutes of the start of the game in Hardcore mode (lists all 16 bosses).
Divinity- Reach greater rift level 75 solo.
Lionhearted- Reach greater rift level 75 solo in Hardcore mode.
Curses- Kill 350 or more monsters in a cursed chest event at level 70 on Torment X difficulty.
Stars Align- Kill 350 or more monsters in a cursed chest event at level 70 on Torment X difficulty in Hardcore mode.
Edit: Boss mode/World's apart conquests are for all 16 bosses in the game.
You should run the whole story (campaign) mode just to know what's going on in the game, but if you don't want to do that then there are ways around it (and you can always run the whole thing later).
Someone else who has completed story mode (pretty much anyone) can invite you into an adventure mode game (and just let you run it until you leave the game- doesn't unlock it for you playing solo) or invite you into a campaign mode game at the very end and you can just kill Mal and unlock adventure mode.
If you need help join a clan or one of the communities and ask for help.
I don't think the fix is working 100% in 2.4.1 but there are a lot of ptr bug reports about it so Blizz should hopefully get the fix on by the time it goes live (fingers crossed). Innas is dropping as the monk seasons bonus set so hopefully they'll take a closer look at it.
When I was doing the set dungeons in 2.4 innas would drop health doing almost anything... it would drop the 6 piece set bonus if I just looked at another piece of gear that could replace a set item (IDs and salvaging gear), health pools, shrines... good times.
Just in case anyone is wondering where this is... it's close to the beginning of a cow rift at the cow king event (similar to the Jar of Souls event in Act 1).

The damage bonus granted is now multiplicative with other sources of bonus damage
So far looks pretty good on the PTR... works well with Zei's gem, etc.
Right now (season 5) there are better passives that guarantee the same amount of damage bonus without having to worry about a monster getting too close and dropping the damage buff. The damage changing to muliplicative in patch 2.4.1 will most likely win out over some of those passives (like Cull). Jump on the PTR, test it out and see what you think.
Ran lots of vaults and didn't see any pet (menagerist) gobs there, none in gob packs, none from bandit shrines. They might could spawn there, but never did for me...
I've found Lady Morthanlu (goat dude waiving a club and his junk around), Malfeasance (crappy spirit one), and Friendly Gauntlet (like Thing from Addams Family). I play on the NA server, but not sure if/how that translates to PTR. I'm using the goat dude of course.
2 of those pets I found in A1 Cath1 and another in some other random area (probably A4 GOH1). Probably around 1 pet gob for every 15-20 runs if I had to guess. With the PTR drop rate nerfed atm it might be much lower now.
Thrill is a must for pushing grifts... otherwise you don't need it. And if you are pushing you probably don't need TA as much as in other situations so you can swap out Thrill (for pushing grifts) with TA (speed for everything else) and use Ambush 100% of the time.
+ The best thing you can do is farm ubers and get a good hellfire amulet so that you can use 5 passives. Unless you just get lucky you will have to craft many amulets, so don't get discouraged if you don't get a good one right away.
(...And if you get a decent hellfire ammy with Steady Aim on it save it for Season 6.)

I got the butterfly wings from (non-bounty) Maghda and the Helm of the Cranial Crustacean (transmog) from The Succulent (unique elite) A5 Greyhollow Island- Tidal Pools dungeon.
Haven't seen the new gobs that drop pets yet.
