As your character progresses through Sanctuary, they should gradually change (for better or worse) psychologically, as well as physically. If a character talks to a lot of townsfolk, they should be more sociable and cheery, whilst the player who avoids townsfolk may only reply to villagers with grunts or laughs.
Likewise, if a player incurs lots of damage, they should receive scars and the like. If they're poisoned or cursed, there should be a physical indication.
