Not sure if this has been mentioned yet but a theory developed in my mind after watching the commercial. Going to try and spoiler tag it, so you can avoid it if you want...
It appears that at 0:09 it is Imperius being knocked back towards the Diamond Gates. But knocked back by what? Well later on at 0:17 you see Tyrael about to thrust what looks like Imperius's spear into something. Normally we see Tyrael with his sword, so it is odd to see him using a spear, even though he threw his sword into the Worldstone at the end of Diablo 2. You can see a similarity in the setting or background in both clips (both large golden halls). Both Imperius and Tyrael appear to be fighting someone or something (what we do not know). If the spear that Tyrael is holding is indeed Imperius's spear, then I think it's safe to make the assumption that Tyrael battles Imperius at some point, and judging by the clip at 0:17, kills him with the blow we see.
Eh, already been mentioned, but not quite in such detail.
I downloaded the video so I could look at it frame by frame in certain areas.
The new footage starts out somewhere in a room in the Silver City.
Then there is Imperius being knocked back; it is unclear just who or what he is fighting in front of the Diamond Gates.
We get a partial view of Diablo curling his hand and moving his arm to attack something or someone. (It should be noted here that the Silver City can be seen in the background.)
The crystal that adorns the Diamond Gates shatters.
Tyrael grabs -- yes "grabs", I watched the frames -- Imperius' weapon, Solarion. He then thrusts the spear towards someone, most likely Imperius. Note in the background that it seems to be the same room that we saw in the "unintended" footage from the Diablo Retrospective video.
That's pretty much the gist of it. Once the full version is on YouTube, I'll do the same for it what I did earlier. Knowing my luck, I'm betting I will get beaten to it.
I'll continue the spoiler just in case people don't wanna know but its kind of an obvious thing to talk about.
I don't think its Diablo's fist. I'm almost certain that is Mephisto's fist. Small correction but still very meaningful I think, so we can expect to see all the prime evils returning perhaps?
I'm pretty sure it's Diablo, bud.
So Diablo grew another pair of arms, his rib cage is split open and is bulimic now? Not saying it is Mephisto that would just be my best guess, but Diablo doesn't even resemble that. Now I know he can take on different forms so to speak but tell me why you think that's him.
Is it just me but I felt that was not a good commercial at all.
Not just you, I was letdown as well (although I read the reactions on here first before I watched it so I got overly excited). I was hoping to see new game play... I don't really care about cinematics... but obviously other people do
I have not read everyone's comments but I skimmed through some of them and it looks like people spotted the same things I did. I posted this on the forms just now ill post it here as well.
And at the :12 sec mark that's clearly a image of the Diamond Gates shattering. If you have the Book of Cain there is a picture of it on page 57. Its Identical to the image in the book. The camera angle is viewing it vertically making it look horizontal. Its really facing up ward showing the gates being broken.
The Diamond Gates protect the Silver City and mention in the book that "no enemy has ever set foot past the gates and into the Silver City" Until now! Hell must invade Heaven because now with the latest Wizard trailer from IGN you will see around the 2:51 mark a Angelic pillar and other pillars in the shot. I looks like Hell with all the fire and red but I doubt they have Angelic pillars in hell?
Call me crazy but the facts are there! Im still so stoked and I just wish I was not obsessed with the game and look at all this little details..... I need a life.....

Torso looks nothing like any of the Diablo art I've seen blizzard do, how are you getting that from it?
Touche! When did Diablo grow another pair of arms o.o....
