As for the plan the prime evils might have had. Look at what Izual says when he is killed.

So it was not Azmodan and Balial who overthrew the prime evils, but it was the prime evils' plan to be exiled to the world.
Could it be that their defeat was all part of the plan? Baal tricked Tyrael into destroying the worldstone.
What I don't get, however, is why they waited 20 years to begging their invasion?

Is from the 'Prophecies of the Final Day'? I thought it first appeared on the Diablo III website and was not included in this prophecy at all.
Makes sense, as the mortal realm wouldn't see it coming considering that they've nearly forgotten the events of D2/LoD. They could also be amassing their armies in hell.
You also forgot the last bit, that is in my sig. In the teaser trailer, there is a scene where a comet, a single comet, is seen falling from the sky.
I'll give you that most of that prophecy tells of the events in D1/D2/LoD, but the last part about the comet... it doesn't tie in with D2 or LoD, but it fits awfully well with the teaser trailer and what we've seen so far.
What's really debatable is whether or not the entire events of D1/D2/LoD were part of an even greater plan by the three prime evils.

The last couple of lines simply mean:

Diablo and mephisto are defeated
"And the last of them would set his sights on the holy mount. The warnings held that Their defeat would be illusory - that the final gambit had yet to be played . . ."
Baal set his sights on mount arreat. The defeat of Diablo/Mephisto was illusory as Baal is still free. Also, the destructions of the soul stone means Diablo/Mephisto are no longer bound to the mortal realm. The final gambit is Baal's march to Mount arreat in order to break the barriers btw hell and the world, thus letting his borthers return.

Baal (Lord of Destruction) marching from Kurast to Arreat
"Reaches out to undo the workings of the Ancients. "
That is, to go pass the guardians of arreat( The ancients) and corrupt the Worldstone.
"The tides of Hell surge - ready to smash Down upon the shores of the mortal world - to drown the guilty and the innocent alike."
Now this part is interesting, as it hints to the end of the civil war on hell, since "The tides of hell surge-" meaning that hell is united, ready to attack the mortal realm once baal corrupts the wolrdstone (which is "the shores of the mortal world"). So maybe the prime evils had a "master plan", to get their soul stones destroyed and then Baal would corrupt the worldstone.
However, this whole prophecy is about what happened from d1 to d2 Lod and not about what will happen in d3. Else, why would they wait 20 years to unleash their hordes upon the world?
