Looking for a challenge? Try beating any Final Fantasy game without dying.
The first one was the only Final Fantasy to really present a challenge, and the remakes of it have changed that. I wouldn't consider Final Fantasy to be a challenging series. Of course, that isn't to say they're bad, 4 is quite possibly my favorite game. I think 1 and 6 were also awesome, the rest were good, but not amazing.
Extreme G Racing 3 for the GameCube. It's a racing game that gets ridiculously fast and ridiculously hard in the higher levels.
I think I've got that for PS2 laying around somewhere. It did get pretty tough towards the end with all the engine upgrades. I never could quite beat it, but my neighbor didn't seem to have any trouble.

I loved both of those as a kid but don't remember them being all that hard, just a little puzzling at times. Which for those that like puzzle solving games, I'd also suggest the NES version of Shadowgate, at least I remember it fondly.
For a challenge on the NES go with Contra, Ghosts and Goblins or Battletoads. Those are all pretty tough. Most people needed to use a cheat for extra lives on Contra.

But yeah, Ghosts and Goblins was pretty freaking hard. Even for people that don't want a big challenge I'd still suggest it just because of the general charm it had. It really was a fun game.

For an RPG on NES, the first Final Fantasy was actually reasonably tough, though the remakes would lead you to believe otherwise.
I can't think of any games that really gave me trouble on N64 and PS2 aside from maybe some fighting games at max difficulty. They just haven't been making that many hard games since NES. The Perfect Dark suggestion isn't bad though, it was a fun console FPS.
