Most of those monsters are on the list because of their location in the game (Act 1). The statistics only count the gross number of deaths, and since the majority of HC characters don't make it out of Act 1, the numbers of deaths to Act 1 monsters are heavily skewed. Doesn't have anything to do with underestimating the foe.
And why all those people die in A1 if not because of underestimation? I believe you can't create a HC char until you kill softcore Normal mode at least once, so there's no way all those people die because they're completely inexperienced and don't know what they're doing.
I don't know how fallen maniacs didn't make the list
1. Because unlike zombies and skeletons, they're never underestimated.
2. Because (according to Blizzard's statistics) like 80% people die in A1 Normal before even encountering maniacs.
IMO, the most dangerous HC killers are:
1. underestimation of danger / overestimation of own ability to survive
2. fatigue
Me (barbarian, wizard) and 2 of my friends (DH and monk) started playing HC Inferno like two weeks ago. We killed softcore Inferno before major difficulty nerfs, so I think it's safe to say we have some experience in this game. We thought that experience will help us evade the most common mistakes people make in HC.
Yesterday my lvl 52 DH friend decided to play solo a bit before going to work. He died fighting a pack of reflect + waller + desecrator spiders. The thing is we usualy stomp such packs into the ground without much problem when play in a group, so he thought it'll be just as easy solo and decided to Gloom-tank instead of kiting them carefully.
Later that day me (on my barb) and my monk friend were clearing Hell A2. We both use super-defensive builds with as much saves, buffs and "-% damage" debuffs as possible, so everything went smoothly. We were playing for like 2 hours non-stop. At some point I realized I totally lost focus and stopped paying any attention to what's going on. I stopped evading fire/arcane/plague crap and stopped watching my HP level - partly because everything was so easy, partly because I was tired after a long day at work. I guess the same thing happened to my friend, because he died recklessly face-tanking a pack of arcane + desecrator + something melee snakes right in the middle of a fire & arcane mess.
What this all tells me:
1. avoid playing HC when you can't pay full attention to the situation
2. make short breaks at least each 30-40 minutes - port to the town, get some tea/coffee, do something else for 5 minutes then get back
3. if you see something at least 1% nasty - start kiting towards the nearest portal through the clear zone
P.S. Yeah, I know I wrote a fucking book and I guess I said nothing new there. But I just wanted to share my thoughts on the matter and obviously I can't discuss it with my friends right now, since they're upset.
P.P.S. Well, at least stash and gold is not wasted and my wizard can AOE-train my friends' new DH and Monk back to 50+ quite fast, so life goes on.
