So I've been thinking all along since the cinematic that the monster they show is a unity of three beings, the middle head obviously being Diablo. Now that the new artwork has been released though it makes even more sense to me.
The project was named "Hydra" which are generally serpentine monsters of legend with multiple heads. It seems in Blizzard's conception, Hydras have 3 heads.
The monster in the cinematic also has three heads.
Note the circled part where you can see the back of the neck of the head on the right.
Now let's take a look at the new enemy, Triune Cultist.
Now Triune by definition seems to hint at the possibility of a unholy trinity.

My guess would be that maybe it's a entity consisting of the three prime evils. Or maybe other beings, one thing seems certain though, Diablo is in there somewhere.
I think the Hydra word for some also referred to Blizzard's 3 unreleased projects: Starcraft 2, WotLK, and D3.

Also the Triune Cultist isn't a monster, it's an enemy.

It would be cool to see either of those things. Since the defeat of the Prime Evils and their subsequent disappearance, you could picture the mortal religions in Sanctuary taking on a form more akin to the Triune in the Sin War trilogy, since the Prime Evils have been reduced again into a more mythical status.
And as for the Cathedral of Light, for all we know the Cultists in Diablo 3 worship Inarius.

This has been discussed plenty of times before, but looking at Blizz penchant for ridiculously big shoulders I think those "heads" are just in fact part of Diablo's shoulders and they have mouths just to make him look more creepy.
As for the Triurn returning, I think there might be some twist to it. Like instead of worshiping the three prime evils maybe they worship Azmodan and the 7 deadly sins. I'm saying this because of that demon the Thousand Pounder who has the title of Gluttony Incarnate which suppose that we'll probably fight the other ones before finally fighting Azmodan.

'Cause yeah, the Triune Cultist must be from the Temple of the Triune, which is located somewhere near where the Witch Doctor is from (southeast of the map).
I'm a bit skeptical of the Three combining into One, which is strange, cause I usually agree with you.. but we'll see.
Yeah i dont see who it could be either, Well technically Lucion wasnt sent to the void, he just became...."nothing". So i guess he doesnt exist at all.
The thing is there are ways out of the voiid if he was actually sent there
The three-headed beast in the Diablo III trailer, I believe is to be Diablo's most truest form. It's terrifying, it's strong, it's ready to kill. I'm sure once he is resurrected, his powers are replenished 100% this time.
Or maybe when Diablo comes back to Hell, he punishes Azmodan and Belial by having them bound to him - therefore unable to banish him away from them ever again.
I just think its diablo by himself, baal and mephisto will return also and the three will plan to take over once again. The mountain that contained the world stone,( i forget its name ) but its going to be the gate way to hell. The meteor is tyrael simply entering sanctuary, it is still probly hard for angels to get to sanctuary even after the worldstone is destroyed because its magic has not faded entirely. So tyrael travels to sanctuary to fight the prime evils along side the humans but his journey dosnt go as planned and the magic that still lingers from the world stones makes his journey difficult and he crashes to earth. little harm has come to tyrael but the meteor is him simply passing through the barrer.
I donno just a thought but it seems to me even before the world stone was destroyed it was much easier for demons to enter sanctuary then angels. So this is my opinion on the matter and tyrael is not corrupt.

I think one of my first posts stated that i thought that he bound azmodan and belial to him as punishment. it was awhile ago before we knew the 3 headed demon was diablo. But mabey those 2 are bosses now that we fight in hell its self.
