Mine was the original Everquest in 1998. And no, I don't have beta access to D3
This was also still early-ish on, where we were all under NDA. I like the nostalgia of their concern for the ability to download the -massive- 250mb client!

Subject: Welcome to Phase 3
Greetings!
You have been selected to be one of our testers in
our Phase III test of EverQuest. We need a few extra people,
and you meet our criteria as having a fast enough connection
to download the EverQuest client software (which will total
around 250 Meg or so).
To obtain EverQuest, ftp to ftp.989studios.com.
Your login name is p4equest and your password is sgeq1057
(case is important!). Once logged in, set binary mode (binary)
and go into the pub directory (cd pub). Download EverQuest.exe
(get EverQuest.ext), then logout (bye).
Make a new EverQuest directory and copy EverQuest.exe
into it. Run EverQuest.exe. This will download the rest of the
necessary files (the EverQuest.exe program is only around 85k
or so).
Once you have logged in, use the following account key:
2fW8uT9F2d2xQZYPgxJ4cYsP5
Please remember your participation in this Beta
will be under the Non Disclosure Agreement you
see when running the program. Please do not
disclose your ftp username or password to anyone.
Also, please begin this as soon as possible. We need
your help in stress testing our servers.
I can hardly remember what my first beta test was. I've surprisingly been in a lot of betas(Not really including open betas) in the past few years. The furthest away beta test that I was in was for Section 8. Though I believe I was in one before that one. But recently I beta tested Rift, SC2, SWTOR, Ghost Recon Online, Counter Strike: Global Offensive, and Diablo 3. None of the beta access for those was because I pre-ordered the game. Though I am an Annual Pass holder and am waiting for my MoP beta invite.

I don't always burn. But when I do, I use hellfire.
I`ve never been into beta testing (I wasn't even looking to get into the d3 until last week). They gave me access to the SC2 beta, a game i didn't really cared about.
I never got any beta key and I have pretty much entered all contests and giveaways I could find.
Same here. The irony is that if I got in to just one I would probably be signed to all future betas because of how much feedback and bug reports I provide in the games i play. If I'm not mistaken they tend to favor good contributors on future betas. My friend use betas as a game preview thingy, he never reports anything and is generally pissy and whiny off as soon as anything is broken. His beta feedback (if any at all) usually consist of a long whine post of how sucky and broken everything in the game is on the general forum. He has gotten into four Blizzard betas so far (WC3:TFT, WoW: Wotlk, SC2 and D3).

well my first beta i played was the open beta of wow right before it was released - played a nightelf hunter till about lvl 15 than wow was released :P.
however my first/only invite to a beta wasn't even through the main site, it was from this fan site forum contest(D3 beta). not only was i extra thrilled cuz it was a D3 beta - also cuz i managed to win something :)(something like a lottery). i couldn't breath for about 10 mins and it was more or less at midnight so i wanted to go to bed while downloading the client - i was so excited i couldn't sleep :P.
My first beta was for Aion. I had a guild that went into the game with me so that was cool. Then we slowly started to die out as we realized how awful the game really was.
Got into the SC2 beta but it's not really my type of game so I didn't play too much of it.
The very unpopular (at the time) site GameStop gave away 25 Warcraft 3 keys. I was a huge Warcraft 2 fan and was looking for Warcraft 3 news when I stumbled across a link to a sweepstakes there. Surprisingly, I won.

Siaynoq's Playthroughs
Then my life got twist turned upside down and I unplugged from the WoW matrix 6 years later. What a ride.
Should have taken the blue pill.

anyway that was my small amount of luck
best regards,

To be honest, I really would like a Path of Exile beta. That game looks seriously fun.

http://www.gamestop.com/gs/wc3/sweepstakes.asp was the old link, but Wayback Machine can't deal with GameStop's outdated form of cookie verification.
Probably the only really special thing I've ever won in my life.
