It is definite that the three-headed demon in the trailer WAS Diablo, having assumed yet another coproreal form.
The power of the three is diminished while in the mortal realm, forcing the to assume a host, which is slowly twisted into an approximation of that demon's form. The "true form" of the three is evanescent and ever changing. For example, Mephisto himself is an ephemeral mass with suggestions of corporeal elements, eg tentacles and clawed hands. This was translated in Sanctuary as a floating mass of rotting and decaying flesh, enshrouded by a thick miasma. The young Prince sustained an arguably smaller, weaker, and less scary manifestation of Diablo; the Khanduras Warrior manifested a far stronger, bulkier version, as befitting a man of such physical, arcane and spiritual strength. The latest physical form of Diablo will likely result from the possession of an even stronger warrior (perhaps the very Barbarian that slew him? Naw, that's been done... HE POSSESSES TYRAEL!!1 =P).
It is stated, however, that there were many sacrifices made to Mephisto. What can be made of this? Was their life essence used to feed him, or fuel the Hell's Gateway? I wonder if it were possible that they were sacrifices in preparation for him assuming an even stronger form...
EDIT Now that I think about it, perhaps the Three wanted to destroy the Worldstone after all. This would allow humanity to regain their lost Ancient powers, allowing the brothers to possess even more powerful mortals to champion their cause against the Heavens! And then Imperius comes crashing down from the skies, to burn sanctuary in a manner reminiscent of Tassadar burning the Terran worlds in StarCraft!!!

Though Belial, the Lord of Lies has not appeared in any of the Diablo games thus far, Blizzard Entertainment has said that one of the two yet unseen Lesser Evils will be featured in the game, which means that the other will probably be soon to follow at some point in the life of the third game. Belial is also said to be in a battle against Azmodan over the control of Hell.
If this is correct, then the two Lesser Evil's are not dead.
I would assume that one will be in D3 Vanilla and the other will make an appearance in the D3 Expansion.
I'm just gonna throw my random idea out and let you break it down. So, we know a meteor hits the church in Tristram. The Diablo we see in the cinematic trailer, to me at least, looks kind of alienish. Did he come down from space on the meteor? Or is my inner sci-fi geek destroying all Diablo logic? After all, there is no space in the Diablo universe. It's Heaven, Hell, and Sanctuary. Although, now that I think of it, if the meteor came from the Heavens...it may have been an angel. This theory is full of holes, but what kind of diablo fansite would this be if we didn't bring up every last strange idea?
As to the Diablo at his full strength thing...The one we saw in the trailer makes the other forms look like goatmen. However, if Diablo was at his full strength nobody would be able to beat the game because he would look at you and you would burst into flames. I think blizzard is just gonna make him look even more badass. If they do give him crazy epic powers they will have to do the same to the player.
