During the 20 year peace, hell has already raged war against the heaven. The fallen angel is perharps tyrael or another angel who was fighting in the heaven and has fallen from fighting the demons. Then the angel would come to the mortal world to ask for help.
Ummm....Hell can get to heaven without going through sanctuary. The conflict has been going on for far longer then sanctuary has ever existed and I am certain there have been countless assaults on the high heavens.

but heaven sealed itself off from hell and diablo never found a way to get there
heaven's strategy was to just wait until the demons turned on each other
which azmodan and belial both did to diablo mephisto and baal
but since neither of them wanna stay dead the sin war is pretty much eternal
what i dont understand is, wouldn't the prime evil's main goal is take back hell, from the lesser evil ? i mean, if diablo 3 is about hell vs heaven, or anything related to hell, wouldnt it be unrelated since hell is not really on diablo's side anymore. It would be more like diablo vs hell vs heaven vs human ?

In answer to lineup's question: The Prime Evil's plan all along was to be kicked out of Hell. They allowed themselves to be exiled to Sanctuary so that they could trick Tyrael into using the soulstones against them, which they then planned on corrupting, escaping, and making their way to the worldstone. Izual was the one that told the Prime Evils about the soulstones and how to corrupt them. Izual states that himself right after you defeat him in-game. But yes, I imagine now that they seem to have achieved their goal of corrupting/destroying the worldstone, their next move would be to somehow find their way back from the abyss and take over Hell once more.

Full-scale military assaults on all the forces of Hell? Not likely.
Not to mention the problem of what the hell is going to happen if Heaven disappears. The whole world of Diablo basically requires the duality between Heaven and Hell. One can't lose over the other, that just doesn't work as we presently know the Universe.

The Nephalem aren't necessarily stronger. They have to potential to be, and even if the worldstone is gone, it will take time for humanity's powers to return. Which definately hasn't happened by the time of D3.
Well ok, our player characters may perhaps be described as a new generation of Nephalem, but that's something we don't know anything about yet.

I thought the Prime Evils and Lesser Evils were already unified, well with Diablo anyway. Duriel and Andariel both helped Diablo in his quest to free Baal to meet with Mephisto which is actually helping the Prime Evils itself.
Plus, when Diablo entered Hell and we followed after, Hell's minions are already allied with Diablo attacking us and crap, eventualyl leading us to defeat Diablo.
maybe during the 20 year peace, the prime evils or whovever you think it is were creating a massive army to attack sanctuary with. maybe sanctuary is a kind of gateway to the high heavens. so if they took over sanctuary they could get to the high heavens in an easier way. and probably during that 20 year peace diablo and whoever else were working behind the scenes corrupting people and whatnot. that way when they got to sanctuary they might have their army hiding all over sanctuary and the meteor in the teaser trailer was a signal for a war beginning. well this is just a random guess for the storyline.... so... yeah im gonna go to sleep now. im really tired.
