I have been watching this over and over, and this is what I can gather happens:
From what we can gather, the girl's name is Leah.

Scenes in order of appearance:
Leah lights candles at night. Judging by her gasp at the end of the trailer, she is performing a ritual that allows her to see into the future. She predicts the coming of the meteor from the Heavens.
Leah has many books with her, and one of the candles sits upon a skull (appears to be an animal's or maybe a demon's skull because of its shape).
There is a huge city next to a river and on the side of a mountain, as well as near other mountains. The city also seems somewhat ruined. And while we see it, she says "...shadow crawls across our world".
- Note that Ureh is a city on the side of a massive mountain (Mt Nymyr), there are also other mountains nearby, there is a river next to it, and the city is currently in ruins. Also, the city comes alive when the mountains shadow crawls across it.
- Conclusion: This city appears to be The Lost City of Ureh! We might be visiting the famous Ureh! That might also mean that we will get to see Zayl the Necromancer.
(Ureh is the featured city in The Kingdom of Shadow novel by Richard A. Knaak, and can be found on both the DII and DIII map).
There is another city in the desert that has access to water. Probably Caldeum.
A comet/meteor flys down directly to a city of what appears to be New Tristram's Cathedral (at night).
Soldiers look on to the ruined site of Mt Arreat (where the Worldstone was destroyed). There is a gaping hole in a mountain, and smoke clouds full of lightning are emitting from the hole. It seems to be on fire.
An army of demons marches/runs across a battlefield with siege weapons and various 'camp fires', while a strong orange light glows from the ground behind them.
Tyrael is seen throwing his sword at the Worldstone, about to destroy it (just like in DII).
A split second of the army is shown.
A split second of the comet is shown.
Leah stands outside of Tristram's Cathedral at night as the comet approaches (evidence by the comet's light lighting up the area).
Leah is inside the Cathedral as it is falling apart, with a strong glow of light coming from outside.
She runs into the Cathedral but keeps falling and slipping as the comet approaches.
The demon army is shown briefly again.
The destroyed Mt Arreat is shown again.
Something firey smashing into the ground is shown (probably the comet) - Note that in the concept art, it is shown that the comet creates a deep hole in the ground.
Demon army shown again.
Deckard Cain is sitting at a table with many books is shown. He is inside a church or cathedral, and for a split second his face and the room light up orange.
- This could mean that Leah runs inside the Cathedral to save Deckard Cain from the comet.
- Note that in the DIII story, it is said that Cain revisits Tristram's Cathedral to try and figure out the source of the recent evil that has apparently been plaguing the lands.
Something fiery and hellish is shown. Like a big crater in the ground. It almost looks as if Leah (or another figure) is on the edge of the crater, with their head sticking up over the edge.
A large demon is shown. It has three mouths. May or may not be Diablo. (please do not discuss this demon here).
Leah gasps as she apparently comes out of her clairvoyant trance.
Leah is the girl in the cinematic and she's somehow working with Cain. At least thats what it seems like considering she directs the cinematic and the Barbarian tells Cain "Leah worries for you" in the gameplay cinematic as he tells Cain to get back to town.
Yea looks like the place is either some meeting place for the demons (I doubt theres many people around there nowadays so it would be pretty easy for them to build up in relative secrecy) or a portal (more evidence for Tyreal conspirators :o) atm I'm personally going with the meeting place as I don't think Tyreal would open up a portal for demons to come through, and if it is a portal, its not what he intended imo.
Sorry for mentioning all that Tyreal stuff, PLZ don't turn this into a tyreal conspiracy thread...
I don't get one thing though. If that was a vision of the "future", why show Tyrael destroying the Worldstone? Did she see this before the events of D1/D2 took place?
Note that this is a trailer. They grab the best bits and peices from different cinematics and throw them together. So don't count anything in it as being in order or related to each other. All we can do is take each individual scene to predict the type of cinematics we will get to see in the game.

I think Tyrael is the meteor, because the background to blizzard.com/diablo3 looks like the cathedral (I have it for my wallpaper now and I just noticed that, lol).

Maybe Baal is included? Because if you pause at the scene of two guards and the arreat crater, and look at the smoke (the upper right area) you can see baal's face like it was after you kill him and his sprit leaves his body.
More like Sescheron or Bastion's Keep. I'm pretty sure Harrogath was on Mt Arreat, while Scosglen was a little distance away. Thats where the Barbarian that gets exploded in the cinematic was standing.

I could be completely wrong, but you can return after the mountain explodes and the NPCs talk about the world stone being destroyed.

He says the worldstone being destroyed is a problem.
So if it's ON Mt Arreat it must of been on the side that didn't get blown to pieces or something.
In fact I'm pretty sure Harrogath was near the base because on the way to Mt. Arreat you have to go through the Bloody Foothills.
