There is no other logical reason for the additional two ehads, unless he has some sort of attribute that gives him unique personality traits.
I think this is his true form; from the abyss he spawns himself his original manifestation; one that does not mimic the textures and colors of the living things of Sanctuary.
If it truly is Diablo himself then I suppose the better question to ask would be "how did he regain his body and why has it taken this form."
Diablo has no "true form". He takes the shape of what represets terror to you. It's not a "lizardlike demon" or "beast like". It's pure terror. pure strength.
And, he doesn't have "heads". Have "mouths". And, again... why not?
EDIT: There is no veil over Sanctuary, so demons have no need to spend energy to keep wandering in the human world (as tyrael does).
His spiryt could be summoned from the abyss, and brought back to the mistycal place that represents tristam.
We never saw Diablo's body. He always posesses human hosts.
I don't know what the need is. Perhaps it is to keep his surroundings stable. Maybe if he has a form even more terrible than what we have seen, that it could annihilate all he touches or something.

I just remembered that they said that Diablo III will conclude the story! So basically, this means that A) We kill Diablo for good! or Diablo wins and rules over or destroys Sanctuary! How can he be killed so he wouldn't come back? I suspect there will be a great twist in the lore!

Yes, there is a difference between killing something and sending something to the (sometimes) eternal abyss. Perhaps we have always killed Diablo's host or physical being, and it is banishment we need, not death.
i reckon it is what the other thread suggested the 3 primevils being brougt out again essentially being re-encarnated, with dialbos head in the middle cause he was the 2nd to die and the main demon or maybe the middle head is mephisto cause it seemed he always told the other 2 what to do (well it did to me).
Sorry if i sound a bit cocky, it's not my intention. I have some troubles to write in english, so finding troubles to express myself really depress me. Sorry. Hopefuly, with time, i will get used to the writing...
I hope it is Diablo. It would be cool to fight him in the same place I killed him in D1. What else could be the "harbinger of Doom"? I guess it could be a corrupted angel...
If it's Diablo, I like it. It's his true form. His most powerful form. The two extra heads look sweet and terrifying. He should be more of a challenge this time. I just wonder if the Lesser and the rest of the Prime Evils have changed appearances...
I didn't read all 50-something pages, but I came across this post and thought I might offer up something interesting. I'm pretty sure this isn't Diablo, and it certainly isn't his "true form". In the Act 4 cinematic of Diablo 2 Mephisto says "Now my young brother, the time has come to assume your true form. Arise, Diablo, Lord of Terror." In this quote Mephisto refers to the Diablo 2 version of Diablo as his "true form". That's not to say that Diablo can't have a new form in Diablo 3, but I think it's more likely that this is a different demon, or perhaps that combination of the 3 Prime Evils that people have been theorizing.
Oh man, that would be so great! I wonder if we'll visit any familiar places in Hell!
I like the area in Hell where the bridge extended just before you descended into the Hell Forge level, just something cool about bridges in hell. I think you could call them bridges to nowhere. When you come across a bridge in hell, you might say Well! What the hell is this bridge doing here? ...and who in hell would take the time to make it? It must be some kind of a damned joke!
I also like Diablo's lair too.
