Because you're basing everything on the Sin War trilogy.
If The Sin War books are considered canon lore for Diablo by Blizzard Entertainment, then the game will use that lore as its base - it would be almost ludicrous not to.
Even if you only used the games themselves, without the influence of external lore, there is still nowhere at all that says Diablo and Baal didn't have children; but I would assume that none of those children were Nephalem. Plus, there was nowhere in any of the games that gave any indication that Mephisto did have children.
I would say she was Baal's blood. IMO it is definitely not Lilith.
Well, looking closely at the Mistress of Pain in the picture found here: http://www.blizzard.com/diablo3/media/artwork.xml#52, we actually can find out some interresting things. First off, she has multiple ways of attacking us, which denotesthat she could be a boss (the usuall enemies usually only do one attack, maybe 2 if you count the horned demon from D1). These include a slashing attack from her forelimbs (what have you), the abillity to launch explosive projectiles that disperse in a cluster around the player, and finally the abillity to emit a sticky sort of spider silk to impede the player. 2nd of all, as spiderlike as she is, she's not exactly an arthropod, as instead of a carrapace or exoskeleton, she has metal armour and clothing, and an external ribcage that help protect her mostly normal flesh.
There's one other thing I find interesting. In the picture found here: http://www.blizzard.com/diablo3/media/artwork.xml#39, she is chained up, yet in the aforementioned picture she is not. Anyone else think there is a story there?
Anyone here seen Aliens vs. Predator. The first one? The Alien Queen was originally chained up then freed. Always makes a character seem more bad ass if the are chained up at one point then released. Either on purpose of by accident.

It does remind me of Astroga, Diablo's creature. I don't think there would be just one Spider thingy lurking about... And being that it's not a child of one of the Three, I think it could still be created.

From the gameplay trailer it looked like they're turning every uniques into a boss fight against a badass demon. It just makes you wonder what an actual act boss will look like.^_^
