After the greatest adventurer of the past detroyed the prime evils the arch-angle Tyreal had to detroy the corrupted worldstone. Doing this released the nephalem into the world and unleashed their true evil. The nephalem began to seek out Tyreal to corrupt him and bring him to the firey pit of hell where he belonged.
The nephalem corrupted Tyreal and sent him to do their biddings. Slowly, Tyreal was sent to seek out the once who had defeated the prime evils. During the time Azmodan and Belial began their own seperate plots to take hell from the other. Also, heaven refused to help the mortal realm now that knew what had happened to Tyreal with all his 'help' in that so called disgusting place.
Tyreal had found the hero in a safe haven on his own to deal with the things that the hero had seen. Also the hero's own insanities. One day while the hero had been suffering from reaccurances of the past Tyreal intruded his sanctuary and struck him down. Only to be torn apart and made into a more powerful being then ever before...a demon.
The body of the old warrior, barely surviving the death of his possesor Diablo, that had struck down the lord of terror the first time had also found safe haven. Also only to be struck downby Tyreal and made into a demon. Both heros and the now corrupted Tyreal set out for new bodies to once again ressurect the prime evils into the world.
The heros and Tyreal struck a deal with Azmodan and Belial to gather together and finally earn a powerful rank in the burning pits. Together the five of the evils set out to destroy heaven and once again bring the prime evils back into the world to corrupt mortals once again. During the battle, the two so called heros corrupted three week angles and brought them back to hell. During this time, Tyreal knowing that Azmodan and Belial could not be trusted, Tyreal struck down the two rivals only to be burned and had their souls wander the mortal realm hoplessly.
With Tyreal knowing how the soulstones were created and corrupted, he created a new and improved one. Made of the blood of many demons. If the soulstones were to be struck by the hammer, the soul bearer or prime evil of the assigned stone, soul would corrupt the destroyer in a more painful way then he or she could even imagine.
The final sacrifices were made for the improved soul stones. These sacrifices were the lives of the three that had helped destroy the prime evils first of all. Given willingly ofcourse. When the soulstones were to be nearly stolen, the sacrificed would come to the mortal realm to guard the soulstone.
With this nearly seeming impossible, the mortal realm once again made an alliance with the angles. All attmepts to recreate failed. Accept one. There was a child. Naturally if seemed addicted to the history of how the prime evils came to there demise. He ofcourse was the long lost offspring of deckard cain. The new heir to the horadrim throne. With the horadrim recreated, the forces of the light created a new leader. Much stronger then Tyreal and a much better leader.
Just before the prime evil the forces of light were ready to wage a war against the forces of darkness. This is where your story starts. Wether it follow the dark or light, may the best of wishes be with you.
When you start you are in the pandemonium sanctuary where you find a gate the the upper sanctums of hell. You enter the gate and fight your way to find a gate to kurast being guarded by a demon named Ganduel who is guarding the gates to hell so that the soul stones may be completed.
In act 2 you are in Kurast whe nyou go threw another gate in hell. Kurast is completely corrupted. All that stands is Cain's son and Meshif. You go and find another gate to upper hell where you find Sosandiar an angle that was once again helping humans. Once you fere him a demon, Jugarsidan comes and tries to stop you, Sosandiar only half chained breaks free once you weaken Jugarsidan half way and destroys him. Sosandiar now acts as Tyreal in the second game and guides you by given you information and the sort.
Meanwhile Cains son and Sosandiar write 3 scrolls to ward off the prime evils soul from corrupting you. You find the materials while they make them. Thus, completed and successful you journey further on your quest.
From here you go to Meshif and travel to Lut gholein. Here you must stop Tyreal from completing Mephisto's soul stone. There you find that tyreal is near complete and he sacrifices himself just before you destroy him and turns into mephisto. Here you go back to town for help and have 2 followers help you destroy Mephisto using the scroll afterwards to keep Mephisto's soul from corrupting you.
Now that no demons are corrupting Tyreal and the Nephalem have past away due to finally completeing they're mission, he returns to heaven to once again be triumphent in his very own throne. You go to find the first hero who will create diablo.
The second hero is possessed by baal. He has already sacrificed himself and by doing so brings Baal back. Baal creates a barier for his soul stone of the screams and terrors of the hero. You get there and try to grab the soulstone only to be trapped by the barrier. Then Sosandiar comes and you play as him to destroy Baal.
The first hero has already been possessed by diablo and is ready to be that such again. You go and find just the soulstone in a large pentegram made of dried blood. The soulsstone is just lying there. You walk over to it and don't pick it up. Instead you feel the ground shake. From the crevices of the ground the lord of terror raises from the ground. You fight him and destoy him, onyl to learn afterward that the scroll didnt work. Instead the soulstone falls into the burning lava near the pentegram and is melted releasing Diablo's spirit to roam the world hopelessly seaching for a body even though he know, he can not possess it.

Ok, I think you have the "Nephalem" wrong here. The Nephalem are, in essence, humans capable of using magic. There's nothing innately evil about them.

Heaven would not do that, since it would be strategically idiotic to give up Sanctuary to Hell. They would fight, just as they always have.

The first hero is dead; Diablo destroyed his body when he transformed in the cinematic after killing Mephisto.

Why would the demons souls wander hopelessly in Sanctuary? There's no rime or reason to that, and neither is there to Tyrael defeating both Azmodan and Belial.

Please, can we let go of the Soul Stones now. I've seen it in two games now, I don't want them again. Heaven knows they don't want them, Hell knows everyone else is aware of them and has no use for them, and the humans can't do them anyway.

A leader much stronger than Tyrael? Tyrael is an Arch-Angel, and as far as we know there is no position above that in Heaven. How, and why, would Heaven make such a warrior?
And Deckard Cain's son? I'd rather not play as him.
