i think stuff in this game is going to be too "dull". what we need is something that really spice up the game, and i mean really. things that can make the game a blast instead of a grind.
the controlled pvp arena is ok, but we need some type of world pvp where dying has some real consequence. for example people can join your game and interrupt your farming by killing you and taking both your gold and your inventory items (not chest or equipped).
this way people would team up with their buddies to hunt other players. this solves the problem of group play being inferior to non-group-play since you wouldnt risk not being in a group.
the details can be worked out, and can even be quite different from what i have described.
the point is just to make the pvp a real battle that matters and is fun, instead of being in "controlled environments" without risk.

edit: rly dont think its gonna be fun... imagine u are workin, having a wife i child.. finally u have 30min-1hour off and wanna do some quick farm or just enjoy the game for a while.. this will ruin it and the game too...
what u wrote is easy... group up or dont play... wanna farm solo? sry not possible, u have weak equipped friends? sry no farm, there will be uber equpped xxx who can gank u all the day... there are no options...
I would be happy if they simply offered a 1v1 arena. Everyone is talking about it, but it hasn't been confirmed. There's a huge rush when it's just you versus someone else. Much more so than in a 4v4 match.

Totally agree that PVP must give consistant rewards.... Like some loot.
Maybe a bit complicated to set up, regarding a few thing my english is too poor to correctly explain.
If you can leave solo and still make money/gear progress it would be a perfect endgame, if no, a gimmick.
I'd love to see a reverse killing streak mechanic in it; if you kill a high streak player it will give you plenty. It would keep casuals interested, as killing that damn bastard with +500hrs (no harm intented i'm one of those) would give you an excellent reward.
On the other hand, must find a way to makes the +500hrs OP stuff bastard's life more and more complicated as he killstreak (like - 1 teammates each X victory).
2-keep everyone interested
Highly doubt they will ever implement any non consensual PvP into the game, leads to too much griefing (see: Ultima Online, the ultimate in griefing games). While I loved the danger of leaving town not knowing if a guild of murderers and thieves could pop up at any time and I'd have to fight or run for my life, many other people do not and don't want to be bothered (cowards!). Hopefully they come up with something new and innovative that blows us away and brings back people. As of now, I have a friends list of people who haven't played in weeks, I'm the lone holdout
Honestly, this would be retarded.
1) Hardcore chars, should we either be excluded from this system or risk randomly getting killed by another player?
2) One thing I enjoy about playing single player is the ability to pause the game and go AFK, without feeling the risk of being dead when Im back, its rude to make others at the dinner table wait for you. If any1 can randomly join my game, then ofc pause wouldn't work.
3) The fear of asking noob questions in general chat would arise, because some1 could find "oh, he is a noob, free kill" and this would kill the community, the Diablo community is currently really helpfull and nice ATM, lets make it stay that way.
Honestly, there could be a lose penalty in the Arena, but besides that, no risk thanks.
