I've seen a lot of good ideas in threads and interviews with the developers regarding Hardcore PvP. Here are two of my variations:
"Sparring" - Basically you fight until you reach 1 HP. You drop to a knee and forfeit without dying (similar to WoW's dueling system). For Team PvP, you would drop to a knee and be removed from the Arena until next round. (This would give those who want to PvP with their HC characters a way to do so without fear of losing all their hard work.
Too easy you say? Not Hardcore enough? Upon forfeiting the match, depending on your level bracket, you are charged a certain amount of gold which is given to the opponent in exchange for letting you live. (You must have the specified amount of gold on your character before you enter into this arena.)
"Deathmatch" - Hardcore rules all apply, strict leveling brackets apply. If you die, your character is gone. Fights can only take place if both players are of the same level. (I understand some waiting queues would be long, but this is to ensure a better experience.) Several warnings would appear before the player chooses this option for his/her first time to ensure they know what they are getting into.
Upon winning a ranked match, you receive a page in your character's profile page which names and describes all of the heroes that your character has defeated and permanently ended.

Maybe there could also be some achievements for success in these modes as well.
I like these ideas. But, we still need some form of going "hostile" in places other than just the pvp arenas. Dueling is great but surpise attacks really get your blood racing.

And I definitely agree. I think among the dedicated PvP modes they should definitely add hostility and the ability to start a duel with a friendly hero if both parties consent.
