You get exactly 1 Hardcore character of your choice in each season. Each character starts out at level 60 with predefined gear for that class (Blizzard can tune the starting gear for balance between seasons).
You play through the game with monster stats and drops equivalent to Inferno Act 1. If you beat all four acts on that difficulty, you continue to play the game again on a difficulty equivalent to Inferno Act 2. This continues into infinity, with ever increasing monster stats, as far as the player is capable of. Each of these difficulties would be called a tier, f.e. Tier 1 Act 2, Tier 10 Act 1, or T1A1, T10A1 for short.
Ladder rank is based on, in order of decreasing magnitude, which tier you’re in, which act you’re on in that tier, the highest boss you've killed in that act, and how many elites you’ve killed (if you’re tied for everything else). Death of a character does not affect ladder rank, but you cannot advance any further for that season.
At the end of the season all characters are lost and everyone has to start over with a fresh character.
You aren’t allowed to start a boss fight unless you have 5 stacks of Nephalem Valor.
Stash separate from Softcore and Hardcore modes.
No auction house.
No Paragon levels.
Ladder-specific achievements.
Two ladders: Single-Player Ladder and Team Ladder.
Single Player Ladder: No other players. Only followers for assistance. No trading.
Team Ladder: You start a Team Ladder mode game whilst in a party of Team Ladder characters (who have not yet begun the season). When you start the first game, they are marked as your team. You aren’t required to always play a game with those people, but you cannot join anybody else’s games, nor can anyone else join yours. They are also the only people you’re allowed to trade with. The team’s overall rank is the farthest any one of the team members has gotten. Elite kill counts are summed for each team. Hence, if you’re the only one on the team currently online, you can contribute by progressing as far as you can on your own and raking up as many elite kills as possible. As for character deaths… it was a team of 4, now it’s a team of 3 . Don’t worry though, you still get the team’s overall score.
Sounds a lot like regular hc but scaled up and with additional features. Applying some of these ideas to the regular hc game will be enough I think. No need to make it all elaborate starting off at 60 with predefined gear and at Inferno level only. I imagine a lot of people will just die in Tier 1 Act 1 anyway.
Essentially, you want blizz to create new difficulty levels behind inferno (T4 Act 1 will be the next difficulty level after inferno, is that right?). Last time I checked only about 120 people have cleared Inferno HC. I think this only caters to the super hardcores with no real appeal to the masses.
I most likely wouldn't play it. Sure some people would enjoy it, but I'd be bitter over any developement time spent into such, when it could be focused on actual softcore endgame. After we get our endgame in the actual game, sure, why not.
That goes without saying, especially since any softcore balance changes affect this dramatically.

You like?
Personally, I hate it. It's adding too much to the game. You are basically adding a new game on top of the game, but not adding content. It's too restricting and not rewarding enough for it to make it worth the trouble. If you want to make a viable ladder system, it has to be in the confines of the current game.
I just lost my first harcore character (a lvl 39 barbarian) because my internet got disconnected and when it came back...boom, I was dead. So I'm never ever having a hardcore char. again because it's a waste of dozens of hours that can end due to a bad internet connection.
