White items are useless, waste time when accidentally picked up
magic items are inconsistent. Find socketed ring in normal act 2, lvl 15 required. Find same exacty ring in nightmare act 4, lvl 43 requirement...wtf?
Played through normal and nightmare twice on two different character (as well as farmingn ormal and nightmare diablo izual and diablo many times). Not a single unique item (let's face it, they are unique, not legendary). Didn't find a legendary until third playthrough in act 3 normal. (I can understand high level uniques being very difficult to find, but the low level unique drop rates are a sad joke)

Any legendary under level 50 is USELESS unless it has a socket. Due to gems not being level restriced, you just need to find socketed items and put top-tier gems in the, My barbarian was using a socketed dagger with a radient ruby for the first 3 acts, until i crafted a bastard sword with a socket...
Endgame is silly
Named mobs have ridiculous amounts of health. Not so much a challenge to kill them, more of a grind.
Invulnerable mobs that are vampiric... I hate you.
Would be much better if it depended on skill and not so much on how long you can play keep away...

Was running normal solo (at lvl 20) when a friend (level 50) joined. He rushed me through, and I got more xp per kill than when I was solo... Don't know if it was a bug, but it was stupid...

Feels like the developers got butthurt when people complained, and this was their childish revenge.
It would be much better if they use the already designed grasslands and rainbows to make it. Use Diablo 3 style graphics for making unicorns and whatnot, and it could be really f'n cool. Bloody cuddlebears, gory unicorns, and add some undead cows...
C'mon devs... c'mon.
A few more things to add to the list:
Izual on nightmare dropping normal plans for staff of herding... wtf?
Crazy lag, getting hit from across the map...
In-game skill descriptions are dead wrong about distances. One barbarian skill claming something like 22 yards distance for damage... Barely goes 2 yards. How the hell did the deveopers mess that up? It's not just a type either, every skill that lists a distance for something is off by a factor of at least 10...
