Wisdom shall be lost. - Cain will die in Diablo III and Imperius makes battle decisions without consulting the council thus the Arch Angel of Malthael steps down and wisdom is lost among the Angiris Council.
Justice falls upon the world of men - Notice that Justice is capitalized in the middle of a sentence signifying a name, Arch Angels of Justice. Tyrael and Imperius battle it out in Diablo III and Tyrael loses his wings and fall upon the world of men from the high hevens. Aka - (The fallen star is Tyreal) Later in act 1 of the game you will rescue Tyreal.
Valor (The Arch Angel Imperius) shall turn to wrath - This states that he is clearly going aginst the collective decisions of the Angiris Council and battling the forces of hell in an unholy and unhonorable battle putting heaven at risk. At seen from the leaked screen shots heaven has become a battle field.
Hope (The Arch Angel Auriel) will be swallowed by Despair. This can be either a representation of a monster or a representation of the Plains of Despair where she could have been overcome in battle by evil.
Death at last, shall spread its wings over all as Fate lies shattered forever. - Death could be a reference of evil or perhaps a monster as Fate is a reference to the Arch Angel Itherael.
Clearly the propechy states that there will be chaos in heaven and it will tip the balance of war in hell's favor which is why in Diablo 3 you must save sanctuary and ultimately heaven as its been previously stated that mere motal can become stronger then angels and demons.

I actually am not quite sure this is true. While I won't care one way or the other, I was thinking about this the other day. The only evidence we have is the little pic on the leaked list of character pictures, and it shows a black guy in normal clothing, then the same guy in some armor. While sure this is probably true, I was thinking, what if it's the paladin from D2? who says that list was complete, you know?
I thought "Wisdom shall be lost." referred to Malthael disappearing after the destruction of the Worldstone... no one knows where he is.
Justice shall fall... Tyrael is the meteor. He was either cast out of heaven by Imperius or went rogue to be able to help us (if the council finally decided to wipe us out). The battle we see on the cinematics can be either after or before this... maybe it's before he was cast out or after he returned with us.
Valor shall turn to wrath... Imperius goes berserk. Pretty much obvious after seeing Diablo 3: Wrath. It's what Diablo had been planning since before Sanctuary was created.

I think "Death" refers to evil forces in general finally conquering the high heavens and Sanctuary (we know Diablo enters Heaven and we battle him there), and I think Itherael somehow ends up dead. Maybe killed by Diablo himself or any other demon. It seems from the leaked screenshots that Imperius gets killed too. And we know he's not a boss.

As far as i can tell , Azmodan is also known as the Lord of Despair, and noted in the book of cain: "He is by far the most charismatic and seductive of all the Evils."
So what my guess here is , that he will seduce Auriel.

Five others (wrath, greed, sloth, pride, and envy) are yet to be seen, but since we have those line in the Prophecy, I think this can be Imperius' further fate.

I wonder if there is any reason as to why he looks alot like the paladin from the Diablo II commercial.

Nothing like a little demon on angel action...
