Not entirely sure, but I would love a confirmation from someone playing the Beta. The reason I speculate it's generated is because I noticed the "Jar of Souls" quest doesn't pop up on the ui until the player clicks on the item.
Upon further reading, some people are saying it's not entirely random. There's a quest in the beta (A Shattered Crown) where you have to find Leoric's Crown, which is supposedly placed in one of the crypts in the Cemetery of the Forsaken. Being that there are many crypts with only one of them being the correct one, one of the wrong crypts may spawn with this encounter in it instead.
So I guess it's just one encounter in a set of "modular encounters" that has a chance of spawning in that wrong crypt. Sometimes you get it, sometimes you'll get a different encounter.
Its actually one thing I have been specifically watching for in all the play throughs is good examples of "random" stuff in the "pockets" of randomness we have heard tell of. I have noticed the randomly placed "mass grave" mini dungeon, and the "well" and cellars and what not. But I have actually not noticed much in the way of "random events" like I was expecting to see. Like quests and the like.

So it's no longer right or wrong... different encounters may be generated... I really like that... not so great for rushing but the benifits far outweigh the cons IMO.

Did anyone else noticed the amount of health orbs ?
And how big they look when there's 20 of them in the screen.
I can't believe one of the first systems announced is still in kind of a bad shape, it's such a simple thing to make a script to limit the amount of health globes to 5 on the screen at most.
Imagine when you get to the further acts and the enemies do more than just shamble towards you? That could be insane. I love it.

I think it's a bit annoying that the "quest completed" box takes up so much of the screen... while monsters are still alive. They should either delay the "quest completed" until you're out of combat, or make it smaller. I would hate to lose a Hardcore character on Hell/Inferno due to successfully completing a quest, then not being able to see the projectile coming at me.

Imagine them being like zombies around Pindleskin..with fanaticism..with exploding upon death..with a boss mob reanimating them..and Iron Maiden! I'm cruel!
