I want diablo one's feel, but WAY better. (The colours in D3 so far are a little bit too clean, bright's okay, but it needs a little more brown)
I want it to feel scary enough at some points, you wonder if it's a game you should be playing.
I belive hell should be a mix of things. Alot of you people had good ideas and I'll explain how some of them could be used.
Someone mentioned smoke and ghosts that whisper to you. Just imagine when you first enter hell having ghosts floating around whispering you telling you to do terrible things (If you have allies they would try and persuade you to kill him perhaps) Or either they whisper to you and make you go insane and your guy wants to kill himself. Also the physics thing (Also be annoying but kick ass) Just imagine your going threw hell (The levels im explaining would be devoid of life with no demons) Suddenly gravity shifts not only do you got ghosts that are vary persuasive your having to strugle keeping your balance as well. (WHen the ghost whispers to you persinally they should have something so maybe you do want to kill your guy either for the heck of it or they make the ghost actually like actually want to kill your guy) I think it would be kind of cool. Then later on you should go down to a level that is pitch black. You can only see your guy. Then when your running along searching for the next level they should have things move to confuse you by thinking its a demon. Occasinally they should have one jump out at you just randomly. (Not a bid dramatic seen just a BOOM to make you shit yourself) People are on edge wanting to kill themselves in the creepiest most weirdest place they have ever been. Just my thoughts

Most importantly: extremely difficult which basically means make a mistake and be death ==>new dimension of hell for the hardcore mode. Love the idea of ghosts telling you how to revenge them :-) I think it would be a good idea if you character will suffer from the "hellish air" which could be reduced stamina, movement reduction or quicker death if you are poisoned. I think it would be incredible if the bosses get some "hell only features" based on a random table so you'll never exactly know which version of eg: Diablo you will face.
Yeah, I'm with you on the extreme difficulty idea, I wanted Hell to be way more difficult in the other games.
The hellish air idea is pretty cool too, especially the dying quicker from poison. Dunno why but that struck me as a mechanic that should be present in Hell.
And the hell only features ! Love that.
Since the demons are on their own plane of existance, you'd think that theyd be MUCH more powerful and deadly.

Enemies would also enter the battle field in new ways you've never expected. Maybe they bust from the ground and toss your character back or seemingly lifeless environment effects lunge and attack you. Some of the enemies advance in such a contorted way that you wince the first few times you see them.
Effects: Visual and audio need to be from another world. We want the screams, caged people and disturbing imagery, but there needs to be more. To really set a player over the edge and make them realize where they are, or rather, make them realize they have no idea where they are, you mess with their mind in reality. Enemies seemingly come into for the attack but vanish or move right through your character like a ghost. A trapped soul asking for help turns into a demon as you are about to help it. You find treasure leading you into a trap. You add in all kinds of scenarios in which the player is unable to predict what will happen in any given action. Some of the enemies perhaps cast hallicination spells which blurs your vision and you view your allies as enemies and everything on the screen becomes skewed, including the audio.
Hell has to have the effect that you can't wait to get out of there but you can't wait to get back in either. It's gotta be hellish.

There needs to be an illusion room or area of sorts. A grassy outdoor area with a bright sky, flowing waterfalls and a rainbow. It'll be an illusion, a false paradise. It could be a boss-room, or a room with a big fight. There would be an objective you need to complete inside the room. This would trigger a transformation in the room, causing it to become hellish. The waterfalls would morph into blood (blood-falls?) The grass would become a crimson-red color and the sky would become red and hazy. As the transformation took place, you would hear the screams and echoes of tortured souls as well as see trapped souls flying around the room. And then whatever it is you have to fight would emerge. It'll be the "Wtf" room of the game.

Hell needs to be depressing it needs to feel like a place you dont want to be. People should get sick and tired of it and just want it to be over. Hell isent sapost to be a fun place its sapost to be a place where you just want to get out! You should get the feeling your trapped and theres no wnere to go. It should make you kinda depressed and a little off edge. Somewhere where you're sitting there just tryin to find your way out. In the first few levels of hell this is what you feel. Your just sitting back looking lazily at the screen. Then you enter another deeper level and suddenly your attacked on all sides. You dont expect it, while you were in a doze tryin to get out you dont react fast enuogh and they tear you apart. Be cool to actually hear your guy scream or something when your being tortured to death
