I sacrifice Mephisto, Diablo, Baal, and Tyreal, ending my battle step, to ritual summon Angel Eyes Black Devil!!! That guy would be bad as hell. A three-headed demon with fiery blue angel wings ftw.
Kabraxis from the Black Road is often reputed to be stronger than a Prime Evil. But it is unclear what he is up to now though. Also that book is not official canon either.
I thought he described himself as having more patience, while the Prime Evils were more confrontational demons. Krabraxis, over time, COULD possibly amass an army of devotees big enough to take down one of the Prime Evils and his legions, but I don't think he could take on all 3 at once. Especially not by himself.
Now, someone else raised a good point earlier: Darrick Lang. What would have happened to him after the worldstone was destroyed? I mean, Darrick states that he becomes the means by which the demon can return to the mortal realm, meaning Kabraxis was, essentially, bound to Darrick. The only thing keeping Kabraxis from returning was Stormfury, and Darricks willpower.
I'm thinking that with the worldstone gone, Kabraxis can now fully materialise in the world. Meaning Darrick Lang has probably long since met his demise come Diablo 3.
If we assume that the book takes place at around the same time as Diablo 2, and that Kabraxis has been free for the past 20 years, he could definetly pose quite the threat, as he would surely have amassed many more followers than before. I could easily see him being a first act boss of some sort.
Anyways, as to why there aren't anymore Prime Evils: I'm thinking because of the Balance that keeps the world of Sanctuary intact. The Balance dictates that when the Demons gain power, a balance is made with the light. If more Prime Evils came about, than more Prime Angels (The ones who I assume are for the most part in charge of the heavens) would come about. It works both ways, and there's no way to upset that balance.
If, however, the Prime Evils were to be COMPLETLY and utterly destroyed somehow, than I suppose we could assume new Prime Evils would be spawned to take their place.
Blizzard have not stated the any books other than the Sin War are official Canon. So Kabraxis may not be a canonical demon. At least until he appears in D3. Then we hare irrefutable proof he is canonical.

This is Sanctuary. I don't find it too hard to believe that, through some astrological, spiritual, or otherwise unspecified change of events, the opportunity could present itself to create a demon of equal or greater strength to the prime evils. That's beauty of creating a universe from scratch.
As far as Leah taking over for Cain, I've thought that since I first saw the Cinematic teaser. "For I've see what the darkness holds..." There's something to that.
And.. LOL at Thizziest. "Stay a while and..."

Doing that allows Blizzard more freedom. They can just deny all those novels. Besides, it adds on the the secret and excitement.
