I think I see a couple problems. First off, don't include your weapon +min or +max damage in the appropriate boxes, those are already included in the weapon damage range shown on the tooltip. Second, are you using a follower?
I can't get the naked stats to work either, I still end up off by like 10 dps (2326.92 dps if I input weapon stats, int, and no +min/max dmg). Since your weapon has no +damage% it should match to within 0.01dps so I don't know what's going on, and have no idea why your dps would change when you remove and put gear back on. Do you have another weapon you can try? I've heard of some bugged tooltip displays where rings or such seem to give different +bonus damage than they display so I suppose it could be true for a weapon.

I'm not entirely sure what is going on but if you add 60.8125 max damage to your total it makes the numbers work for both gear sets. Can you check the numbers without any of your +bonus damage gear (i.e., OH, amulet and rings)?

I tested further - I sequentially added in my Off-hand, then rings, recalculating after each item was equipped, and the spreadsheet and ingame DPS continued to agree.
UNTIL.. I added my amulet. Then the numbers started to be off. I took off all other sources of +dmg except for the amulet, and still the numbers were off. I suppose there could be something wonky in the way the spreadsheet calculates +dmg on amulets, though I think there is another more likely problem:
I have a Tal Rasha amulet, which has "Adds 6% to Cold Damage" - it is possible that this may be being miscalculated somewhere, either in the ingame tooltip (probably lol) or on the spreadsheet.
Full stats for the amulet are

My simple spreadsheet stats with the amulet but no other +dmg gear are:

Yeah, that +x% elemental damage is odd. I haven't messed with it a lot and so far can't quite figure out what it's doing to the damage number. So far I don't have any functionality for it in my spreadsheet.
using the full equipment calculator, with all buffs set to 0, and no ingame buffs:
all values (int, armour, AR etc) matched ingame and on the spreadsheet. The only value that doesn't match is the DPS.
calculated DPS: 18114.43
ingame DPS: 18712.07
Same paramaters but with amulet removed:
calculated DPS: 15821.89
ingame DPS: 15821.89
With my +goldfind amulet

I'm no theorycrafting expert, but I would say that proves the +cold damage is the source of the discrepancy between the ingame DPS tooltip and the spreadsheet
if it helps i realise there is a difference in the dps number btw
1. equipping the storm crow helmet after wearing the magic source
2. equipping the source after storm crow helmet
Is it possible that the DPS display matters are differeing because the user's weapon happens to have the matching damage type to the helm/amulet's damage increase (+X-Y Lighting Damage on a Weapon with Storm Crow, or +X-Y Cold Damage on a Weapon with Tal'Rasha's amulet)?
This is just a guess is all. Cause the downside to most all Wizard spells is, they do not carry the damage type through to the spells. The lone exception is Spectral Blades, being the only offensive ability we have that just does % weapon damage. All others convert the damage to a specific type, so they don't carry the elemental damage property over. This is easy to check due to +X-Y Cold Damage on a weapon snaring enemies, and that it will only snare with Spectral Blades and nothing else.
Just an idea as to what would have the calculations off, since it would only apply to the bonus weapon damage, and not ALL the damage done by the weapon + stats... maybe...

ther are ring's and armor with a bonus to elite mobs i found out that that bonus goes in the dmg in-game..
if you like i can show some print's i took from my gear and the comparation...
but for me 3 % dmg to elites its like 500 dps more

If I'm understanding the stat correctly, bonus to elite mobs does not show up in your damage number shown in game but instead only applies to the damage done to mobs. So one ring with 100 int and 3% damage to elites would give the same Damage Number as another ring with just 100 int. Equivalently weapons with similar dps values, one wth the bonus damage, one without, will yield the same damage number. I tested and have a screensot to show. Both weapons have the same dps and you can see that equipping the other weapon will not change the Damage. If you want the speadsheet to approximate the effects of that stat, just add the appropiate % in the Bonus Damage box. However, doing so will make the spreadsheet no longer match your in game Damage Number but should give an equivalent DPS on elites.
I believe it is the bonus damage from the ring. I've seen a few cases where bonus damage on rings and amulets seems to not be the actual amount shown on the tooltip. Try increasing the min damage of the ring in the spreadsheet by 1 at a time and see if you can get an exact match between the spreadsheet and in game. If you can find an integer value that gives a match, then I believe there is a tooltip error for the ring.
Yeah, because it is an integer value I expect the tooltip is wrong on your ring. In other words your ring says +19-37 but it is effectively +23-45 or something like that, or at least that is the value according to your damage number. In terms of DPS, +min damage = +max damage and it doesn't really matter where it shows up since all values are added directly to the weapon anyway and then averaged to calculate the effective weapon dps, that's why you could add the 12 damage to weapon damage, min damage, or max damage and still get the same results.
I made a minor addition today in the form of some quick and rough Life Steal/Life On Hit calculations. Basically I got a new wand and was curious what the equivalent Life On Hit would be for that weapon based on my current stats and Arcane Orb.
There are 3 new boxes you can fill in are found in the "Equipped Items" tab: "Skill Weapon Damage %", "Life Steal", and "LoH Coefficient".
The first box is just the weapon damage % of whatever skill you want to look at. For me it's Arcane orb and I don't use the obliterate rune so it does 175% weapon damage. The next box is simply how much Life Steal you have or how much you want to examine. The last box is the life gain coefficient for the rune you are using. For convenience I have copied and pasted the approximate coefficients found on the battle.net forums and provided a link the the source thread. I use Celestial Orb rune so my coefficient is 20%, found in the "LoH Coefficient" tab.
The results are shown directly below the DPS. It reports your average hit from the skill in question, taking into account your crit info and attack speed. It uses that info and the Life Steal value to calculate how much life you gain per attack on average. Then it uses that info and what the LoH coefficient is for that skill to give you an equivalent LoH amount needed to equal the Life Steal. The idea being you can compare the two to see which will benefit you more or to simply compare the two values (LoH vs Life Steal).
Use this information at your own discretion. It likely isn't very useful to many people but for those using a kiting spec and shopping for a LoH weapon, hopefully you can use this to see if you're better off getting a life steal weapon instead, which would hopefully be cheaper. Those using a melee type build might see greatly differing results depending on the skills used.
7-17-12 Added logic box for Archon form to display EHP while the ability is active. Equipped Comparison tab also takes the box into account, but remember that EHP only applies while the form is active

It's because you deleted the Max Weapon Damage in the Equipped collumn. Also, you don't have to input the same gloves into Item 2 since you already entered it into Equipped, but it is a good check to make sure it's worring correctly as it should give 0 dps change since it's the same as equipped.
EDIT: Also, to use the "Equipped Comparison" tab, you have to enter your own gear in the "Equipped Items" tab, otherwise it will be comparing the stats of items to my current gear and use my own dps stats.
so in short, it looks like it doesn't anticipate the min+max dmg from offhand waepons.

Try putting the OH damage in the "Min+Dmg" and "Max+Dmg" box instead of the weapon damage boxes. The weapon damage boxes currently only work for the MH weapon while the other boxes work for all items.
i love this thing, but i wish i could factor in my actual LOH and life per second, unless i'm not seeing it. Also since its for a wiz, i would like to know if there is a way to incorporate some kind of APoC slot. haha
