256 LoH - I bet you die a lot. Or you're only playing low MPs or whatever. Bump LoH.
28k HP, 28 life% - not a good choice, replace all of this with vitality (it's only about 170 vit anyways). Life% is a waste below ~35k HP.
Your armor is too low, try to get some more armor on any piece you intend to replace.
These are the most critical weak spots of your char. Your CMWW stats are fine (2.56 APS/19 APoC/50.5% CC), you can leave them for now, but you should seriously bump you survivability stats - all of them. Try to aim for ~30k HP by replacing life% with vitality, some 4000ish armor, maybe one more piece of allres, and 400-600 LoH. Yes, this will definitely cost you DPS (unless you have shitloads of money), but it'll make your char much more stronger. I have no idea about your budget, but I don't think you're super rich, so I'm just gonna throw some suggestions at you here which might be in your price range:
Get a Blackthorne's pants. Gives you LoH and some decent EHP. Don't even need to go crazy here.
Replace your right ring (the one without IAS), because a ring without IAS is a bad ring for CMWW. You can even try to get the missing EHP here, like almost all of it - just make sure the ring has high CC and good IAS.
After replacing pants and ring, replace your amulet. You want IAS here, too, and also a bit higher CC. (If you manage to get 11% IAS on ring+amulet combined, you'll make it to the 2.73 breakpoint, go for it).
There are obviously some things you could improve about your other gear pieces as well, but most of them would be costly. If you need to sacrifice a stat in order to get more EHP, it would be life% (because your HP is so low, vit is much better) and crit hit damage (CC+IAS are much more important for CMWW).
