Bad Medicine works well with poison darts for another defensive passive since item wise I go more damage

Bane of the Trapped - Great constant damage proc'd by Frozen Piranahas, your stun, level 25 proc on gem
Enforcer - Increased pet damage and survivability
Gogok of Swiftness - Darts from fetishes are shot at the speed of your attack speed and easy to get 15 stacks creating more consistent damage and a lot better than pain enhancer on smaller groups/bosses
Explanation:
The carnevil dart build has a lot of utility with stuns and good wave clear so I wanted to see how viable it was. Had a friend say he can only take it to 30 so I tried it out. (Edit: he said he only went to 30 and that he could go higher but didn't) Since your using the carnevil combo you get stuck using the Zunimassa set which is kinda meh in terms of limiting the rest of your build compared to using a starmetal pet build. I used Zuni Chest, Offhand, and boots since you need the helm slot and rings for unity/rorg. The rest of the items are to your liking. I used Cain's glove and pants for the additional attack speed. Aughild's set for shoulder and bracers for the damage and survivability. I only had like 10.5m toughness? Belt I used witching hour since its the best damage item in that slot for the build IMO.
The playstyle is face first and a lot of fetish managing. So you need to pay attention to projectiles and things on the ground. The fetishes will lag behind as you attack as you move. To make them move up or closer to enemies you just have to stop attacking and they will move up to melee range. There are times where the fetishes will shoot the wall or something that you can't do much about though in some situations. Like attacking to keep procc'ing the stun, life per hit or bad medicine. You also can run away to reset the fetishes by having them spawn on you by running backwards/forwards.
To me the survivability of fetishes takes precedence in maximizing your damage. That's why you select so many defensive passives as well as for yourself. So there are times where you have to stop attacking with darts to spirit walk to horrify elites which stops them from doing their affixes or to protect yourself. Horrify is great to stop things like winged assassins coming in or using it aggressively with spirit walk to stop the green monster that sends out worms in the ground that will destroy your pets hp in high grifts. And why horrify over big bad voodoo is cause the cooldown is too long and you keep higher damage with pets alive longer.
Spirit walk. The key to this build for me. You use it to stop reflect which is around 3 seconds up and 12 seconds down. So you can spirit walk to either continue to aggressively attack while reflect is up or you can stop attacking. Spirt walk is also great to dodge affixes/monsters or use it to run in to horrify things. You also have to sometimes spirit walk to grab health globes to heal you pet.
For stats on your gear I went with the basic crit/chd/atk spd on gear. Accessories I went mostly crit/crit damage. My Cain's gloves are trifecta with int/chd/cc/atk spd. The only piece with +fetish damage was my Zuni's offhand cause I just used whatever was in my bank. Rest of the gear was defensively rolled cause I wanted a okay base toughness.
Alternatives and Toys:
Haunt of Vaxo - Dart to the Face is one of 2 skills we can use to proc it
String of Ears, Hellcat Waistguard, Fleeting Strap, Blackthornes neck & belt are all decent replacements if you don't have witching hour
I wanted to fit Ancient Parthan Defenders for the stun damage reduction passive but fitting it made my base toughness lower but could be interesting

2. Why not Efficacious Toxin?
3. Why not Swamp Land Waders?
4. If your dogs are life linked where does your healing come from?
Well everyone has their own playstyle so if you find that you like the healing bbv thats good for you. I just use horrify cause it has use on all packs and I'm on hardcore.
1. Pain enhancer doesn't increase damage of fetish darts but it applies a dot on crits. It something I considered for the build but the problem is when you have a smaller pack or the rift guardian you'll only have 1 stack which means only 3% atk spd. With gogok you always have 15 stacks which is 15% atk spd (additive) and the fetishes shoot darts as fast as you shoot them. The little bit of cdr can also help.
2. No efficacious toxin cause the dot takes too long to tick and I like the damage/benefit of the other 3 more. Enforcer increases pet damage consistently by like 22 something percent (additive I believe) at rank 25 I forget and gives them more survivability. Gogok gives you consistent 15% atk spd which is important in this build to me. Attacking faster means more darts which means more damage more chances at stunning. Bane of the trapped works well with the dart stun, frozen piranhas, and the level 25 slow the gem gives. Bane of the trapped is also something that's easier to manage yourself. I don't like bane of the powerful cause in higher grifts you run into packs that take a while, don't have the buff for the first pack, or the buff could fall as you might not be able to find an elite between the timer.
3. No swamp land waders cause I took Cain's set bonus as a priority. You get 8% attack speed that will make up for the 15-20% poison damage. Additionally the 15-20% poison damage gets traded off for more toughness on the pants. I was at 10ish mill toughness which isn't that preferable for me at grift 35.
4. You still get life back from your dart. I had a little bit of healing on something + paragon points in life on hit.
Would TnT be bis for this build? does the glove's ias applies the speed at which the fetishes fire their darts?
As FearArzel intimated. do they apply only to knife attacks? if not, then it would be much better to use 2 piece from Cain's/Asheara/Jade sets to utilize its bonuses.
