I can't complete a 58 at the moment. My gear's still missing an ancient Fist (20 rerolls and counting...), but otherwise seems pretty decent to me.
I don't die too much (2-3 times max), mostly because I get careless, standing too long in multiple aoe or getting charged by a bogan, but I don't kill things fast enough.
I skip yellows and try to get blues in large packs, but I feel my main problem is pulling enough mobs to me, if I move over a screen away they just don't follow any more ...
You have almost no CDR... You should aim for close to 60% at least, before Gogok 15% kicks in. That makes your Epiphany come up much faster, also you can "spam" SSS more with higher CDR.
Also, to much gear with AR, you need secondary resists on more pieces. Lastly, change Sixth sense to Determination/Mythic Rythm for damage boost. Since you say you are kinda OK on survivability.

Regarding CDR, I'm at 49.88%, I know I need better F+R but they just won't drop.
Regarding secondary resists vs AR, same thing, Kadala's been a bitch. What should I look for instead of AR, on chest/legs though? Quin's guide says LPS, is it really that good for the build?
Chest options are lps/armor/reduce damage from elites. Legs, LpS/armor.
LpS is tricky, though, would be good to stack a lot of it to make it viable. Also, GR60, maybe go with Molten Wildebeast Gizzard for LpS instead of Stricken, and damaging passives, just to see if it works out better for your current state of gear.
I know your probs not going to like this advice, but get a better hellfire passive, guardians path can be traded out for more dmg picks.
for example get a "must" passive - Beacon of Ytar/Near Death Experience/Harmony - this then gives u the flexibility to choose mythic rhythm and determination if your lacking damage to push.
I thought Guardian's Path was included in most U6 builds? At any rate, I'm much more fragile w/o it, so I'd rather keep it. I'm usually running Beacon of Ytar/Near Death Experience/Harmony/Determination + TGP on ammy.

Gameplay-wise, at which GR level did you feel you had to start fishing for rifts? Or at least when did you feel you couldn't always get them, depending on density and yellow spawns? Currently working on 59, I've been noticeably ahead of the timer on the first floor only to find half-empty 2nd and 3rd floors, or tons of yellow very close to each other.
Guardians path is used in basically all higher end Grifts. GP, Mythic Rhythm, Beacon, NDA, Harmony. Correct use of Mythic Rhythm makes up for the loss of Determination or other dmg skills.
The only down side is for farming you have a Guardians path neck. Beacon or Harmony is more flexible if you are going to swap specs etc.
Well, thanks to some fairly sizeable upgrades (ancient Fist of Az, a very solid Restraint, a better helm and finally getting that 15% EP on boots), I just cleared a well-populated 60 with time to spare (around the 11'30 mark).
Time to finish upgrading those gems for the season journey and then try to push higher
I think you're running more into gameplay or RNG issues than gear issues. I did 62 back when i was wearing 4 ancients (both weapons and 2 armor), and I wasn't dieing. You shouldn't be dieing any times as a Uli monk. By the time NDE procs, you should be playing safe, and in most cases skipping past whatever just killed you. The chance to get to the end of a GRift and running out of mobs is really low.
Most of the time you should just run past the elites, which is generally the only things that can kill you in the first place.

Still, getting a second ancient weapon did wonders for both my dps and survivability. I went from a 2180 dps Fist of Az without vit to a 2700 dps with vit. Basically allowed me to switch 300 points of paragon vit into dex, while still gaining significant toughness.
Most of the time you should just run past the elites, which is generally the only things that can kill you in the first place.
I definitely should, but I still get greedy sometimes.
I'd like to chime in and say that I got stuck at 58 and am now working on 64. The difference was when I chose to fight rather than simply a gear check. Do not get baited into small packs. You must find or create the density. Density = more damage = faster clears. Do not finish off packs. Pop the EP, clear the majority of them, and move. I am still learning to do this.
