One thing I notice is that you have a lot of wasted stats on your gear. I think you should replace the items with high intelligence and strength. They give you protections but you are only benefitting from 10% of their value.
I did it guys. Was like you said. After passing the second stage, the third was the same as the first. I was dying a lot more for my greed to try banging him than hit and run. The moment that I understand the most important thing is hit and run as much as possible I passed the second stage and reaching the third round for the first time, I could beat him.

I was just about to suggest to you and other dh's struggling on this to try out Rain of Vengeance with the shadow beast rune for the Shadow Clones. It really worked wonders for me in getting the kill. Smoke screen just before the clone comes up, pop rain on the clone, move away and dps it.
And also as someone else said.. stay away from edges. When he doesnt port like a madman he is really rather trivial to fight.

Congrats! Ph1+Ph3 is a total joke. You can "bang" it out, but I found the safer route probably the best approach because PH2 is a nightmare with some bad RNG.
PH2 = pop raining vengeance/stun+stand there and nuke him with NT/SS spam. Run around for full disc, hatred, cooldowns before 66% and 33%. Watch minimap for Diablo's icon to disappear to know that your shadow clone will spawn.
In all phases, stay away from outside edge, fissures/cracks, mountains, etc. to prevent diablo from teleporting. You can kite him all day until you're ready.
