I think the proper way to weigh this really depends on what you're trying to do. What kind of content are you doing that requires you to already be running Ignore Pain? T5? T6? If so, Pride of Cassius can be invaluable if you refuse/are unable to to abuse Unity rings. If you are using Unity rings, Ignore Pain and the Pride of Cassius are both overkill.
Now if you're doing lower torment levels and are running Ignore Pain, I might reconsider doing so. Especially since you have a String of Ears available. If using String of Ears means you won't be using Ignore Pain (for lower torments), that's an entire skill slot left open for you to choose something useful. Consider that.

I only play in full grps and in T5. i never play solo, thats just way to boring for me.
That Pride of Cassius's legendary affix is on the low end, only +4 seconds. That being said, if you use Iron Hide and some cooldown reduction, I'd go for the Pride of Cassius.

As far as I know, all elite affixes are also considered as either melee or ranged attacks, so string of ears should work on some of them.
Not 100% sure, but I've read that all the ground effects are considered as melee, while mortar and arcane enchanted for example are considered as ranged attacks.
I guess unless you are using Ignore Pain with a heavy CD reduction build for nearly perma usage, I would prefer the String of Ears.
