I'm not complaining. Please don't interpret this as whining. I am currently running my lning barb (+40% ish lning dmg) through T1 bounties and absolutely killing it-until I get to Malthael. My current build/gear setup runs through other act bosses and elites, but the big bad angel of death stops me cold.
Is there a baseline resall/healing you need to hit to do that fight? I'm only posting because the slide in difficulty from T1 Diablo to T1 Malthael seems super steep...
With out seeing your current build and gear its hard to say what the problem is. He can be a tough fight, but if I move out of his clouds and AOE i can stay up with the occasional health pot, he goes down pretty fast.

No. Drop him down to hard and don't hit him, just watch him. Learn his attack patterns and work it from them. He only has one mostly unavoidable move, and it's also his least damaging one.
Edit: After you've figured out his attack patterns, die to him intentionally, leave game and make new on higher difficulty.
What was said here is pretty accurate. Avoid standing in clouds and dont stand on him when he's doing his death siphon. Beyond that the really hard part is in p2 where your goal is to get behind him when he shoots out the fireballs so that you can avoid that devastating lighting blast he does. (It hits everything except for behind where he's standing)
Your two offensive attacks are going to be Weapon Throw+Balanced Weapon and Ancient Spear+Jagged Edge. It helps to have an item or two that increase your fire damage because of this. The one passive you definitely need is No Escape for the 25% bonus damage. There are more than enough instances in the fight where you can get the 20 yard distance for the damage increase. Don't throw anything at him while he has his shield up though, the projectiles will just bounce off (and possibly smack you in the face).
Relentless is a great talent here because it can save you from a lot of damage if you get low, and can also enable you to spam Ancient Spear. I'd recommend War Cry+Veteran's Warning just because it provides the most toughness in almost all circumstances (15% dodge is huge when you have no dodge), and the damage boost from Battle Rage is nice too. I use Bloodthirst, but it generally requires a pretty good amount of health globe healing bonus (a few 70 items will do). Superstition is great too because of the tons of non-physical damage being dealt.
I use Leap to stun to enable Haunt of Vaxo shadows to come out, but you can change that to whatever you'd like (I'd keep it being a fury generator skill though, instead of say, sprint). Call of the Ancients are pretty good too, because they don't really die while they're out, but I only use them because I have 4-piece bonus. If you don't, you could probably use something else.
I'll link some build stuff and my cobbled together crappy gear later. I am in no way well geared. I'm one of those players who's just making the best possible damage/survival ratio he can with what he's managed to get thus far. I guess I was just surprised at the difference.

This is crucial. I play a demon hunter, so maybe I am just way squishier, but this one-shots me on T1+. His 'blood wings' cover the entire room except for the dead zone directly behind him. I've noticed that the first time he does this move in phase 2, he faces southward, so the spot to get to right off the bat is directly north ("above") where he is facing; each subsequent time, he seems to face in random directions, so you have to pay attention to his model, but he always faces that one direction the first time he does the move in each fight instance.
You have to be fairly methodical to beat him consistently. The methods to dodging his various attacks are pretty straight forward, you just have to account for the few RNG factors like the bullet walls, cloud placement, and wing positioning.
What used to be my downfall fighting him as DH was that I was impatient. I would try to power through his HP and push him through phases, and I would get hit with stuff that was perfectly avoidable. When I slowed down and start paying more attention to the mechanics and less to his HP bar, the fight got a lot easier.
One tip that might help is that, if you know you can't avoid some of his damage, then think about where your next heal is going to come from. If your potion is on timer, then situate yourself within range of one of the health wells, or stall until he's in a phase where you can outheal his damage via LoH, or kill his adds but leave their HP Orbs on the ground until you need them.
I do have to say that the Act V bosses are significantly tougher then any previous act's boss. Besides all 3 of them easily giving anyone a beating, they also have a significantly larger amount of health, which is probably why thier burn-achieve is much longer then the previous bosses. (3 mins for Malthael kill compared to 1 min or less for Act I-IV.) I don't even bother with Act V bounties anymore, the amount of time spent on the bosses isn't worth the reward.
I consider Malthael to be equivalent of at least the next difficulty up of any other boss, if not 2 difficulties up. IE: T1 Malth is about on par with T2 Adria, maybe even T3.
The difficulty ramps up dramatically for that fight and that fight alone. Release night, my friend and I managed to get to 70 and beat everything up to Malth on Torment I (I was carrying him fairly hard). The second we hit Malth, we were getting wrecked. I ended up dropping the game and defeating him on T1 solo, which took some spec tweaking including redoing how I spent Paragon points, but my friend had to drop the game down to hard difficulty to finally beat him. I just couldn't make up the damage my friend was lacking to carry us through the fight. He's a monster and a very difficult yet extremely fun fight.
My advice to you is Health regen is your friend. One thing that really helped me was I changed my Paragon points in Defense from max All Res to max Life Regen. That 8000+ health per second was a godsend. Beyond that, adjust your spec for more survival. When I solo'd him on my barb, Warcry and Threatening Shout were a must. Then it's just practice, learn his routine and beat his ass. Good Luck
