I've been playing as a Demon Hunter and im halfway through act 2 inferno. I've run into some pretty difficult mobs on the way, some of them have some pretty messed up combination of affixes. But this thread is more about individual affixes. Which one do you see and just say 'oh shit' and just know its going to be a hard fight?
-Fast: enemies are exceptionally fast and can get on top of you, and stay there with ease.
-Mortar: Makes it so you can almost never stand still during one of these fights, if you do, youre dead.
-Reflects Damage: even when the enemy never touches you, you're dying. fast.
-Jailer: You're a sitting duck without a spell to break root!

Voted fast. A combination of all those would be hard though:p Fast Mortar Reflects Damage Jailer.
Worst one I've come across so far: Fast, Immortal minions, Jailer, Illusion:( SO MANY JAILS AT THE SAME TIME NOT ENOUGH SMOKE SCREEN TO BREAK THE ROOT TOO MANY MOBS ALL OVER THE PLACE RUNNING TOO FAST. CAPS LOCK RAGE.
Nothing is universally bad on every type in every combination.. But I think I would vote for Vortex being the overall worst because it sucks with everything from fire chains, to waller, to fast, to arcane enchanted, to frozen... pretty much makes everything worse. I find Jailer usually isn't a problem unless it is fast jailer or jailer waller.. maybe jailer shielding since if they jail you when they go immune you are screwed.

Also, the already fast Naga in Act 2 that can go invisible and avoid everything until they hit you are by far the most stupid idea ever.
As a DH this is my favorite one, as long as they don't have reflect too. Elemental Arrow slowed down hits the mob 4+ times.
