I want to buy the Book of Cain, but I am worried that it will ruin the Diablo III story line. I know that the Book of Cain is suppose to fill in some of the gaps of the history of Sanctuary, but will it include any events of D3? Does anyone know if the Book of Cain will contain any spoilers?
No not as I've understood it. It's supposed to be the all-encompassing compilation of the story of Sanctuary from it's beginning until, but not including, the events in Diablo 3. We may get some notes from the start of it about it in the way of "Recently a new hero rescued me from the skeleton king blabla", but I doubt it'll be anything major.
I have been wondering this myself, I know the book contains a bit of art but it's mostly story stuff yeah. However since the book of cain comes out before the game itself and blizzard wanted to preserve story by limiting the Beta, I doubt there will be any spoiling stuff.

The book of Cain may spoil some of those little lore books you pick up - but not the core story of the game. The Book of Cain is a history book. Think of it as the Silmarillion of the Diablo World - but probably written less AS a religious history.
It gives you the key events that led to the situation that the main event(in this case Diablo 3) tells of.
Since the book is going to come out before the game, you can bet there won't be much - if any - information about the events of D3.
I believe the whole point of the Book of Cain is for Blizzard to present the definitive (retconned) version of the history of "the Diablo Universe" (for lack of a better term). That way we won't need a whole lot of exposition when we enter the game. I suspect there might be some new (small) revelations that come out of it, but none of them will ruin the story of D3.
Btw, the book is slated for December 13th (source).

Never discard the possibility of it containing very important clues to what will happen in D3, though.
