You can use the Steam controller. I tried at one point and although with that you can map any button as well as the two 'track pads' that act like a mouse to anything you want, it's just not the same as a MK setup. The biggest issue is that unlike the console version, there's a lot of spells that fire at where your mouse is aimed, which screws it up a lot.

When I was recovering from two leg operations over the space of a year, I needed to control my PC from a more comfortable position, I am really good with a 360 controller and played most of my games (lots of skyrim in the day) with controller exclusively. I got a program called Pinnacle game profiler which I then made a basic setup to control the mouse and left/right click. Basically I used this with an on screen keyboard to put together my emails and try to keep my work going (albeit slowly) Then the trial for that program expired just as I was thinking about making it work for Diablo.
I then got a program called Xpadder and after months of trial and error made a really decent controller profile that I'm quite happy with. Of course I'm not as fast as someone with a mouse, but with practice I got very good. These days I cycle on my recumbent bike while playing D3 with a controller, I find it a great distraction and it makes me feel a bit less of a slouch just playing at my desk.

That is because it doesn't support it.
