it doesnt make sense tho, even wit Diablo being the Strongest and the Leader of the three it doesnt make sense of why the looks of the heads of Baal and Mephisto isnt shown, theyre just heads covered wit skin for some reason

Metamorphized as in grew from under the skin and burst out of the human cocoon. If it hadn't of been a full form shift he'd of still had to be vaguely humanoid instead of saur-ish. But considering the other extreme redesigns so far seen, I'm not even sure.
He was still restricted by the Soulstone. What he was from the Soulstone even with a stronger host was nothing compared to what he could be in his True Form.

My bet is Azmodan or Belial ... no way it is Diablo - looks way too different - blizzard would not change diablo that much especially how they drew him in the wwi 2008 logo.
That three headed demon is not Diablo. Diablo has distinct aesthetics which have remained quite similar in all Diablo games.
It is definitely something we have not seen before. So this is a newcomer and perhaps a lesser evil, cause all prime evils have already been featured.

It is quite possible that the Demon is Diablo. This revamped style of Diablo resembles the Divine Comedy's version of Diablo - containing 3 heads. If you don't know what the divine comedy is, then I'll say no more. Just read Dante's Inferno and you'll understand.

1)3 Rules hell and the lesser evils.
2)Lesser evils feel the 3 have relented in the war against Sanctuary; rebel.
3)3 are cast into the mortal plane in their true physical manifestations.
4)They wreck havoc across Sanctuary.
5)Tyrael shows up to the Three Great Mage-Clans and convinced them to join up as the Horadrim.
6)The Horadrim chase down the 3 with the worldstones and slay them; (Whomever slew Mephisto, Tal Rasha and company slay Baal and Jereid Cain defeats Diablo.)
7) They imprison their souls, before they can escape to find a host into the Soulstones.
8) They eventually corrupt mortals and use their bodies to make perverted forms of their true selves.
9) We come along and destroy these false bodies.
Ok, so... there you go. Meaning, the last time any one ever saw Diablo, Mephisto or Baal in their true form was several hundred years ago.
The last mortal to see Diablo in his true form was Jereid Cain.
So get off it.
Mephisto and Baal had neither of their true forms, wanna know why? Here's proof...
The Councilmembers are distorted by Mephisto's soul being influenced on them. Are you saying they're still human? No.
Look at Baal, for christs' sake. He's had to survive in the decomposing body of Baal. It wasn't until he had his soulstone that he was able to "pervert" it to a form more suitable to his original form.
And so far as the "accumulation of power allowing Diablo to rise as his true form" theory goes, that's somewhat easy to explain. He can control a body through direct contact with the soulstone at such a level but because the soul is not actually using the body as a total vessel, he cannot return to his true form. This is what the soulstones were used for; to cripple the powers of the Prime 3 and imprison them into the mortal plane.
So... his original being "thrown" into Sanctuary has his true form, hm.
His powers are limited and handicapped when he assumes his next two guises: oh, wait, he kind of looks weaker..
