After becoming almost convinced that the demon in the cinematic couldn't be Diablo, I decided to search a little. I read most of the threads on this forum and others dealing in debate as to who that demon is. Everyone seems to think it's either Diablo or Belial. Though I couldn't understand why everyone ruled out Azmodan so soon. So after trying to find Azmodan referenced in a religious text or something of the sort, and finding NOTHING, I found this:
"Occultism & Parapsychology Encyclopedia: Asmodeus
Ancient Persian demon of lust and rage who also appeared in ancient Jewish folklore, where he was believed to cause strife between husband and wife. He is mentioned in the book of Tobit ca. 250 B.C.E., where he attempts to cause trouble between Tobias and his wife, Sarah. Jewish legends claim that Asmodeus was the result of a union between the woman Naamah and a fallen angel. Asmodeus was often represented in magical texts as having three heads—a man, a bull, and a ram, riding a dragon, and carrying a spear. Directions for evoking this demon are contained in the well-known magical textbook The Magus; or, Celestial Intelligencer by Francis Barrett (1801).

Asmodeus; Not quite Azmodan, but certainly close enough to bear more than a coincidental resemblance. Notice it is also said this demon had three heads, and was born of a fallen angel, also falling in line with what we've seen/speculated so far (as it's become generally accepted fallen angels will come into play in D3 lore). Now I'm not well versed in Diablo backstory so I don't know if maybe Azmodan's origin has been discussed in a book and goes against this.
Here's another couple of excerpts I thought interesting enough to share: "In the Malleus Maleficarum
In the Malleus Maleficarum (1486), Asmodai was considered the demon of lust, to which agreed Sebastian Michaelis saying that his adversary is St. John. Some demonologists of the 16th century assigned each month to a demon and considered November to be the month in which Asmodai's power was stronger. Other demonologists asserted that his zodiacal sign was Aquarius but only between the dates of January 30th and February 8th.
He has seventy-two legions of demons under his command. He is one of the Kings of Hell under Lucifer the emperor. He incites gambling, and is the overseer of all the gambling houses in the court of Hell. Some Catholic theologians compared him with Abaddon. Yet other authors considered Asmodai a prince of revenge.
In the Dictionnaire Infernal
In the Dictionnaire Infernal by Collin de Plancy, Asmodai is depicted with the breast of a man, cock legs, serpent tail, three heads (one of a man spitting fire, one of a sheep, and one of a bull), riding a lion with dragon wings and neck, all of these animals being associated with either lascivity, lust or revenge.
In the Lesser Key of Solomon
Asmodai appears as the king 'Asmoday' in the Ars Goetia, where he is said to have a seal in gold and is listed as number thirty-two according to respective rank.[2]
He "is strong, powerful and appears with three heads; the first is like a bull, the second like a man, and the third like a ram; the tail of a serpent, and from his mouth issue flames of fire."[2] Also, he sits upon an infernal dragon, holds a lance with a banner and, amongst the Legions of Amaymon, Asmoday governs seventy two legions of inferior spirits.[2]"

Belial makes a better possibility. The demon appears to have 3 mouths.
As Belial is the lord of Lies, one might be inclined to think that one mouth always tells the true, one always lies, and the third randomly, and by random I mean either truthfully or falsely. Not random ans in non nonsensical.

-Humankind cannot gain anything without first giving something in return. To obtain, something of equal value must be lost. That is alchemy's First Law of Equivalent Exchange. In those days, we really believed that to be the world's one, and only, truth.
If you freeze the part where the demon is shown in the cinematic, you can clearly see a gap between the middle head and the right head/mouth. Indicating that at least the one on the right, has a neck. A mouth with a neck doesn't make much sense..
that's why it's fanart, now in the cinimatic we saw a 3 headed demon, so is it diablo, is it one we havent seen, is it multiple demons put together. Who knows, but i like it when it yells.
What is all the debate about? The three headed monster is most likely just a hydra. It looks like a hydra, and looks too much like a typical monster to actually be a significant character. If it were a prime or lesser evil, it would have more detailed facial features, it wouldn't just look like a fiery bug-like creature. I think people are reading way too much into this.

Not even a hydra to me, ONE HEAD, STOP SAYING IT HAS 3 HEADS. Mouths as shoulders YES, but not 3 heads ffs.
After he breaks the world stone the demon souls rush into him and take over, hence the multiple looking heads, weither they are heads or not doesn't matter.
I'd think making him 3 mouthed or whatever they are doing is going to make him pretty original this time around. I don't understand why people are opposing the idea...
In Diablo 3, the prime evils are going to attack sanctuary in their TRUE FROM HELL forms, not from corrupting some mortal and morphing their body to the Evil's desire.
We have not seen their true forms.
Diablo 3 will present their true forms, how they actually look in Hell.
Not to mention, they won't be the weak little peices of shit we fought in D1 and D2.

Beginning to hate all the people complaining about the Art Direction.
maybe not his true forms yet in diablo 3, maybe he took some poor mortal souls altogether to form a great one, maybe he then killed in diablo 3 , and his plan fails again. and thus, the end of diablo series. kk bye bye sanctuary, we wont and disturb u anymore sob sob

We have only see the forms of Diablo that he has taken when possessing a mortal from his Soulstone, never his true form.
kkthxbai
In Diablo 1 and 2, we see Diablo where his soul has TAKEN OVER A MORTAL BODY.
We have not seen
DIABLO AS HE WOULD BE IN HELL WITHOUT HIS SOUL CORRUPTING A BODY.
Break it down.
Diablo 1 and 2.
Corrupted mortal body.
Diablo 3.
Real Diablo
Get it, got it, good.
Buncha derpderp's.
We have not seen their true forms.

and those shoulder-mouths dont look like a bull and whatever azmodan was supposed to have, but he still might be in the game itself.
